锁定加压钢板外固定对股骨远端骨折手术后感染的临床效果

【摘要】:目的探讨锁定加压钢板(LCP)外固定对股骨远端骨折术后感染的效果。方法选择2012年3月-2015年4月医院骨科利用LCP外固定技术结合置管冲洗负压引流技术,对股骨远端骨折内固定手术后发生感染的12例患者的临床资料进行分析。结果 12例骨折患者中,平均(44.00±10.50)岁;首次手术后到发生感染行LCP外固定的平均间隔时间为(16.17±2.73)d;所有患者均完成整个随访过程,随访时间平均为(15.50±1.51)月;患者骨折平均愈合时间为(19.17±2.25)周,全部病例均为骨性愈合,感染也全部得到有效控制;患者在手术后6个月膝关节屈曲角度平均为(110.17±10.66)°,伸直角度平均为(2.83±2.29)°;手术后1年的膝关节特种外科医院评分(HSS)平均(90.17±5.56)分。结论利用LCP外固定技术结合置管冲洗负压引流技术对股骨远端骨折内固定手术后的感染进行控制和治疗,具有创伤比较小,在Ⅱ期并不需要进行内固定更换,手术后并发症少而且节省住院费用等优点,作为一种对感染性骨折的治疗方法,有效性和安全性较高。
[Abstract]:Objective to investigate the LCPs of locking compression plate. Methods from March 2012 to April 2015, the orthopaedic department of the hospital used LCP external fixation technique combined with the drainage technique of irrigation and negative pressure. The clinical data of 12 patients with infection after internal fixation of distal femoral fractures were analyzed. Results the mean age of 12 patients with fractures was 44.00 卤10.50 years old. The average interval between the first operation and the infection with LCP was 16.17 卤2.73 days. All the patients completed the whole follow-up process, the average follow-up time was 15.50 卤1.51 months. The average healing time of fracture was 19.17 卤2.25 weeks. All cases were bony union and infection was effectively controlled. The average flexion angle of knee joint was 110.17 卤10.66 掳and the average extension angle was 2.83 卤2.29 掳6 months after operation. One year after surgery, the average score of HSS in special surgery hospital of knee joint was 90.17 卤5.56). Conclusion the infection of distal femoral fracture after internal fixation was controlled and treated by LCP external fixation combined with catheter irrigation and negative pressure drainage.
