TKA术后患肢屈曲位对隐性出血和活动度影响的Meta分析

【摘要】：[目的]评估TKA术后患肢不同体位对术后隐性出血量和活动度的影响。[方法]计算机检索Conchrane Library、Pub Med、Embase、OVID、CBM、CNKI、万方和维普数据库中关于TKA术后患肢体位对术后隐性出血量影响的随机对照试验(RCT),检索时限为建库至2016年5月,同时查阅所获取的参考文献索引。两名研究者对检索收集到的文献按照制定的纳入排除标准进行独立筛选。采用隐性出血量、总失血量、输血率、术后血红蛋白水平、术后活动度、发生深静脉血栓率作为评价指标。对纳入文献进行方法学质量评价,采用Review Manager 5.3软件进行Meta分析。[结果]最终纳入13篇随机对照研究RCT,1 775例患者。Meta分析结果显示:两组患者在隐性出血量、总失血量、输血率、术后血红蛋白水平、关节活动度差异均有统计学意义;深静脉血栓发生率差异无统计学意义。[结论]患肢屈曲位作为有效且安全的方法对TKA术后减少隐性失血量、总失血量、术后血红蛋白降低量、输血率和增加关节活动度有显著帮助。
[Abstract]:[objective] to evaluate the effect of different positions of affected limbs after TKA on the amount and mobility of recessive bleeding after TKA. [methods] to search the Conchrane Library Pub Medbase OVIDC CNKI, Wanfang and Weip databases about the influence of different positions of affected limbs after TKA on the amount of recessive bleeding after TKA. The search time is from the construction of the database to May 2016. At the same time, the index of the obtained reference documents was consulted. The two researchers independently screened the collected documents according to the exclusion criteria formulated. The hidden blood loss, total blood loss, blood transfusion rate, postoperative hemoglobin level were used. The range of postoperative activity and the incidence of deep venous thrombosis were used as evaluation indexes. The methodological quality of the literature was evaluated. Meta analysis was carried out with Review Manager 5.3 software. [results] in 13 randomized controlled studies, 1 775 patients. Meta analysis results showed that the two groups had occult blood loss, total blood loss, blood transfusion rate and postoperative hemoglobin level. There was no significant difference in the incidence of deep venous thrombosis. [conclusion] flexion position of affected limb is an effective and safe method to reduce recessive blood loss, total blood loss and hemoglobin after TKA. Blood transfusion rate and increased joint motion were significantly helpful.
