应用Ilizarov外固定器行关节牵伸对兔膝骨性关节炎软骨的影响

【摘要】:目的:应用Ilizarov环形外固定对兔膝骨性关节炎关节进行不同程度的持续关节牵伸,观察其对关节软骨组织形态学和MMP-1、-13及IL-1β表达的影响,评估治疗效果。方法:24只新西兰大白兔随机分为正常对照组(A组)、模型对照组(B组)、正常牵伸治疗组(C组)及高度牵伸治疗组(D组)。模型组及治疗组采用关节腔内注射木瓜蛋白酶溶液的方法建立兔膝骨性关节炎模型。造模成功后,C、D牵伸治疗组手术安装特制Ilizarov外固定支架,分别进行不同强度牵伸治疗,治疗时间为6周。实验结束后观察各组家兔软骨组织形态学变化及软骨中MMP-1、-13及IL-1β的蛋白表达变化。结果:组织学检查显示模型对照组软骨的病理损害程度明显重于正常对照组(P0.01),软骨组织中MMP-1、-13及IL-1β的表达也明显高于对照组(P0.01)。而经关节牵伸治疗后,软骨的病理损害程度较模型对照组明显减轻(P0.01),软骨组织中MMP-1、-13及IL-1β的表达也较模型组显著减少(P0.01);高度牵伸组中上诉蛋白的表达较正常牵伸组减少(P0.05),但两组软骨组织Mankin评分无明显差异。结论:应用Ilizarov环形外固定对OA关节进行持续稳定的关节牵伸可延缓关节软骨病理退变,并可抑制软骨组织中MMP-1、-13及IL-1β蛋白的表达,其蛋白减少程度可能与牵伸强度有关。
[Abstract]:Objective: to observe the effects of Ilizarov ring external fixation on articular cartilage tissue morphology and the expression of MMP-1- 13 and IL-1 尾 in rabbit knee osteoarthritis. Methods 24 New Zealand white rabbits were randomly divided into three groups: normal control group A, model control group B, normal drafting treatment group C) and high drafting group D group. The model group and the treatment group were treated with articular cavity. The rabbit knee osteoarthritis model was established by injection of papain solution. After the successful establishment of the model, special Ilizarov external fixator was installed in the treatment group. They were treated with different intensities of drafting, The time of treatment was 6 weeks. The changes of cartilage morphology and the expression of MMP-1- 13 and IL-1 尾 in chondrocytes of each group were observed after the experiment. Results: histological examination showed that the pathological damage of cartilage in the model control group was serious. The expression of MMP-1- 13 and IL-1 尾 in chondrocytes was significantly higher than that in control group (P 0.01), and the expression of MMP-1- 13 and IL-1 尾 in cartilage was significantly higher than that in control group (P 0.01). Compared with the model group, the pathological damage of cartilage was significantly alleviated (P 0.01), the expression of MMP-1 + -13 and IL-1 尾 in cartilage tissue was also significantly reduced than that in the model group, and the expression of appellate protein in the high draft group was significantly lower than that in the normal draft group, but the expression of appellate protein in the cartilage tissue in both groups was lower than that in the normal draft group. There was no significant difference in Mankin score. Conclusion: continuous and stable joint drafting with Ilizarov ring external fixation can delay the pathological degeneration of articular cartilage. It also inhibited the expression of MMP-1- 13 and IL-1 尾 in cartilage tissue, and the decrease of MMP-1- 13 and IL-1 尾 protein may be related to the drafting intensity.
