股骨头坏死20例组织病理学观察

【摘要】：目的探讨股骨头坏死(ONFH)的组织病理学特点,为全面认识该疾病和临床防治提供依据。方法收集本院2011—2012年间手术切除ONFH送检标本20例,复习临床资料,全面阅读切片,总结分析。结果 20例中发生于右侧11例,左侧7例,双侧2例。主要症状为髂关节疼痛,活动受限。平均病史8.2年。病理学检查:股骨头标本以变形为主,关节面粗糙、凹凸不平,骺软骨板厚薄不均或断裂、分离、脱落;剖面骨质实变,呈暗红或杂色,失去正常蜂窝结构;组织学改变分3个层次,即软骨退行性改变、死骨成分和间质反应性增生。由于病变进展期不同,损害程度差异,使得病变组织结构复杂多样,相互交织,病变不仅能破坏正常组织结构,亦可破坏关节累及周围滑膜组织。结论 ONFH是常见病,临床及影像学即可明确诊断,病理组织学的深入观察可为早期防治提供理论依据。
[Abstract]:Objective to investigate the histopathological features of osteonecrosis of the femoral head (ONFH), and to provide evidence for comprehensive understanding of the disease and its clinical prevention and treatment. Results among the 20 cases, 11 cases occurred on the right side, 7 cases on the left side and 2 cases on the bilateral side. The main symptoms were pain of iliac joint and limited movement. The average history was 8.2 years. Pathological examination showed that the specimen of femoral head was mainly deformed and the articular surface was rough. Uneven, uneven thickness or rupture of epiphyseal cartilage plate, separation and shedding; section bone consolidation, dark red or miscellaneous color, loss of normal honeycomb structure; histological changes are divided into three levels, that is, cartilage degenerative changes, Due to the difference of pathological progression and damage degree, the pathological tissue structure is complicated and varied, intertwined with each other, and the lesion can not only destroy the normal tissue structure, but also damage the normal tissue structure. Conclusion ONFH is a common disease which can be diagnosed by clinic and imaging. The further observation of histopathology can provide theoretical basis for early prevention and treatment.
