超声引导肌间沟注射治疗疼痛相关性颈性眩晕的临床疗效观察

【摘要】：目的:观察超声引导肌间沟注射治疗颈部疼痛相关性眩晕的临床疗效,探讨颈部疼痛在眩晕发病机制中的作用。方法:采用随机数字表法将40例颈性眩晕患者分为治疗组及对照组。两组患者均给予常规治疗,包括改善后循环药物、心理治疗;治疗组患者在此基础上给予超声引导肌间沟注射治疗。于治疗前、治疗2周后采用颈交感神经症状评分表(cervical symphathetic symptoms evaluation questionnaire,CSSEQ)对两组患者进行疗效评定,同时记录治疗组超声引导肌间沟注射治疗主观感受评价、眩晕改善起效时间、眩晕消失时间。结果:两组间治疗前CSSEQ评分比较,差异无显著性意义(P0.05),治疗2周后治疗组CSSEQ评分(5.26±2.47)分、对照组(12.84±3.71)分,均较治疗前明显降低(P0.05),治疗组较对照组评分降低更为明显(P0.05);治疗组注射后0.5h内所有患者治疗侧颈肩部胀、热,上肢麻木、乏力,18例患者自觉头清目明、视物清晰。治疗组眩晕改善起效时间平均为注射后(4.0±1.5)h,眩晕消失时间平均为(5.0±1.5)d。结论:定位精准的超声引导肌间沟注射治疗颈部疼痛相关性眩晕有效,消除颈部疼痛是治疗颈性眩晕的关键。
[Abstract]:Objective: to observe the clinical effect of ultrasound-guided intramuscular groove injection in the treatment of cervical pain associated vertigo. Methods: 40 patients with cervical vertigo were randomly divided into treatment group and control group. The patients in the treatment group were treated with ultrasound-guided intramuscular sulcus injection. Before and 2 weeks after treatment, the patients in the two groups were evaluated with cervical symphathetic symptoms evaluation questionnaire CSSEQ. At the same time, the subjective feeling evaluation of ultrasound guided intramuscular sulcus injection was recorded in the treatment group, the onset time of vertigo was improved and the time of vertigo disappeared. Results: CSSEQ scores were compared before treatment between the two groups. After 2 weeks of treatment, the CSSEQ score of the treatment group was 5.26 卤2.47, and that of the control group was 12.84 卤3.71, which was significantly lower than that of the control group (P 0.05), and that of the treatment group was significantly lower than that of the control group (P 0.05), and that of the treatment group was significantly lower than that of the control group (P 0.05), and the neck and shoulder distended and hot in all the patients in the treatment group within 0.5 h after injection. 18 patients with upper limb numbness and fatigue were conscious of clearing their head and eyes. Conclusion: ultrasonic guided intramuscular groove injection is effective in treating cervical pain related vertigo. Eliminating neck pain is the key to the treatment of cervical vertigo.
