不同治疗方法对高龄股骨转子间骨折的疗效分析

【摘要】:目的:探讨动力髋螺钉、股骨近端防旋髓内钉和人工关节置换3种方法治疗老年股骨转子间骨折疗效的差异。方法:2012年9月~2014年3月,分别选择髓外固定组、髓内固定组与人工关节置换组各30例分别接受DHS、PFNA、人工关节置换术后比较3组的手术时间、术中出血量、住院时间、术后并发症、下地活动所需时间和Harris功能评分等结果。结果:髓外固定组、髓内固定组患者的手术时间术中出血量均优于人工关节置换组但下地承重所需时间比置换组较长,3组住院时间、Harris功能评分比较差异无统计学意义。结论:3种方法治疗高龄患者股骨转子间骨折疗效均较满意,但股骨近端防旋髓内钉手术更适合于高龄患者。
[Abstract]:Objective: To explore the difference between 3 methods of treatment of femoral intertrochanteric fracture in the elderly by dynamic hip screw, proximal femur anti rotation nail and artificial joint replacement. Methods: in March ~2014 September 2012, extramedullary fixation group was selected respectively, 30 cases of intramedullary fixation group and artificial joint replacement group were divided into 3 groups after DHS, PFNA, and artificial joint replacement. The operation time, the amount of bleeding in the operation, the time of hospitalization, the postoperative complications, the time needed for the ground movement and the Harris function score. Results: the amount of bleeding in the operation time of the extramedullary fixation group and the intramedullary fixation group was better than that of the artificial joint replacement group, but the time required for the lower ground bearing was longer than the replacement group, the 3 groups of hospitalization time, and the Harris function evaluation. Conclusion: the 3 methods are satisfactory for the treatment of intertrochanteric fractures of the elderly patients, but the proximal femoral intramedullary nail operation is more suitable for the elderly patients.
