基质诱导的自体软骨细胞移植技术治疗膝关节软骨缺损的早期疗效

【摘要】:目的探讨基质诱导的自体软骨细胞移植技术治疗膝关节软骨缺损的早期疗效。方法分析23名(24膝)采用基质诱导的自体软骨细胞移植技术治疗的膝关节软骨缺损患者,平均随访31个月(6~48个月)。术前及术后1、3、6、12、24、36、48个月进行国际膝关节文献委员会(International Knee Documentation Committee,IKDC)评分、Lysholm主观膝关节评分;术前及术后3、6、12及24个月进行膝关节MRI检查,并测量各时间点移植区与正常区T2值。结果21名患者(22膝)获得临床随访,术前IKDC评分为(59.6±14.3)分,术后 1、2、3、4 年分别为(74.2±13.2)、(78.4±10.0)、(78.6±5.9)分和(88.4±10.2)分,较术前有明显提高,差异有统计学意义;术前Lysholm评分为(66.9±15.6)分,术后 1、2、3、4 年分别为(87.2±9.3)分、(88.2±8.1)分、(89.8±5.3)分和(95.3±5.8)分,较术前有明显提高,差异有统计学意义。11名患者(12膝)获得MRI随访,术后1年、2年时移植区软骨与正常软骨的T2值无明显差异。结论在年轻的膝关节软骨缺损患者中,基质诱导的自体软骨细胞移植技术具有良好的早期疗效。
[Abstract]:Objective to investigate the early effect of matrix-induced autologous chondrocyte transplantation in the treatment of cartilage defect of knee joint. Methods Twenty-three patients with articular cartilage defects were treated with matrix induced autologous chondrocyte transplantation. The average follow-up was 31 months to 48 months. Before and 1 month after operation, the International Knee Documentation Committee (ISCC) score and the subjective knee joint score were evaluated by the International Knee Documentation Committee for 48 months, and the knee joint MRI was performed at 612 and 24 months before and after the operation, and the T 2 values of the transplanted and normal areas were measured at each time point. Results the IKDC scores were 59.6 卤14.3 before operation, 78.4 卤10.0 卤10.0 卤78.6 卤5.9and 78.4 卤10.0 for 4 years after operation, respectively, which were significantly higher than those before operation (P < 0.05). The preoperative Lysholm score was 66.9 卤15.6). The MRI scores were 87.2 卤9.3 and 89.8 卤5.3 卤5.3 卤5.3 卤5.3 respectively in 4 years after operation, which were significantly higher than those before operation. The difference was statistically significant. 11 patients with 12 knees were followed up by MRI. There was no significant difference between the T 2 values of cartilage grafts and normal cartilage at 1 year and 2 years after operation. Conclusion Matrix-induced autologous chondrocyte transplantation has a good early effect in young patients with cartilage defect of knee joint.
