【摘要】：目的：观察应用脱细胞异体真皮基质填塞修复高位肛瘘的临床疗效,探讨治疗高位肛瘘的微创治疗新方法。方法：选择我院收治的100例高位肛瘘患者,治疗组采用脱细胞异体真皮基质填塞术,对照组采用传统的肛瘘低位切开并高位挂线术,比较两组手术时间、术后疼痛视觉模拟评分、术后疼痛持续时间、术前及术后肛门失禁程度评分、术后并发症、创面愈合时间、住院时间、一期手术成功率、治愈率、复发率。结果：治疗组手术时间、术后疼痛视觉模拟评分、术后疼痛持续时间、术前及术后肛门失禁程度评分、创面愈合时间、住院时间均低于对照组(P0.05),一期手术成功率及治愈率高于对照组(P0.05),两组在复发率的比较、术后并发症的比较上,差异无显著性意义(P0.05)。结论：脱细胞异体真皮基质填塞修复高位肛瘘的治疗,在降低高位肛瘘复发率、减少术后并发症方面与传统低位切开并高位挂线术方面基本一致；在减轻患者疼痛、促进创面愈合、减少住院疗程、降低对肛门功能及外观的损害、提高治愈率方面具有较为显著优势,可进一步在临床中推广应用。
[Abstract]:Objective: to observe the clinical effect of acellular allogeneic dermal matrix tamponade in the repair of high anal fistula and to explore a new method of minimally invasive treatment for high anal fistula. Methods: 100 cases of high anal fistula were treated in our hospital. The treatment group was treated with acellular allogeneic dermal matrix tamponade, and the control group was treated with the traditional low incision of anal fistula and high ligation. The time of operation was compared between the two groups. Postoperative pain visual analogue score, postoperative pain duration, preoperative and postoperative anal incontinence score, postoperative complications, wound healing time, hospitalization time, primary surgical success rate, cure rate, recurrence rate. Results: in the treatment group, the operation time, postoperative pain visual analogue score, postoperative pain duration, preoperative and postoperative anal incontinence score, wound healing time, The length of hospitalization was lower than that of control group (P 0.05). The success rate and cure rate of primary operation were higher than that of control group (P 0.05). There was no significant difference in recurrence rate and postoperative complications between the two groups. Conclusion: the treatment of acellular allogeneic dermal matrix tamponade for repairing high anal fistula can reduce the recurrence rate of high anal fistula and reduce the postoperative complications. It has obvious advantages in promoting wound healing, reducing hospitalization course, reducing damage to anal function and appearance, and increasing cure rate, which can be further popularized and applied in clinic.
