双镜联合治疗胆囊结石合并胆总管结石的策略分析

【摘要】：胆囊结石合并胆总管结石的传统治疗手段为开腹胆囊切除+胆总管探查+T管引流,尽管该方法疗效确切,但创伤较大。随着腔镜技术经验的累积、微创理念的深入,胆囊结石合并胆总管结石的治疗方法出现巨大变革,现阶段常用的微创治疗方法有两种:腹腔镜胆囊切除术联合内镜逆行胰胆管造影/内镜下括约肌切开取石术及腹腔镜联合胆道镜胆总管探查术,根据内镜治疗的时相、取石途径、胆管缝合方式等不同,具体操作方法也不同。本文作者对各种方法的优点、缺点、适应证和并发症作一综述,为临床上选择治疗方法提供一定的依据。
[Abstract]:The traditional treatment of cholecystolithiasis combined with choledocholithiasis was open cholecystectomy and choledochus exploration and T-tube drainage. With the accumulation of experience in endoscopic technique and the deepening of minimally invasive concept, the treatment of cholecystolithiasis with choledocholithiasis has changed greatly. At present, there are two commonly used minimally invasive methods: laparoscopic cholecystectomy combined with endoscopic retrograde cholangiopancreatography / endoscopic sphincterotomy and laparoscopic combined choledochoscopy with common bile duct exploration. Different ways of stone extraction, suture of bile duct, and so on, the specific operation methods are also different. The author reviewed the advantages, disadvantages, indications and complications of various methods, which provided a basis for clinical treatment.
