ERCP治疗老年人胆总管结石的临床分析

【摘要】：目的探讨治疗性ERCP技术在治疗老年人胆总管结石安全性及有效性。方法回顾性分析2015年3月1日至2016年3月1日在广西医科大学内镜中心行内镜逆行胰胆管造影术(ERCP)治疗老年胆胰系统疾病患者的临床资料,对比老年组和非老年组患者胆石症ERCP操作种类频率、治疗成功率、并发症发生率、死亡率等。结果本研究中236名患者共行ERCP取石治疗267次,A组为年龄大于80岁患者,共行ERCP操作20次;B组为年龄60岁至80岁患者,共行ERCP操作110次;C组为年龄18岁至60岁的中青年组,共计137次操作。A组、B组、C组患者ERCP取石治疗好转出院率分别为100.0%、99.0%、100.0%,完全取石成功率分别为70.6%、75.0%、91.9%。3组患者ERCP治疗有效率、术后胰腺炎、胆管炎、出血等并发症发生率差异无统计学意义,无ERCP所致穿孔发生,总计ERCP相关死亡0人。结论在做好术前评估及预防情况下,ERCP应用于老年人胆总管结石的治疗是安全、有效的。
[Abstract]:Objective to evaluate the efficacy and safety of therapeutic ERCP in the treatment of choledocholithiasis in the elderly. Methods the clinical data of elderly patients with cholangiopancreatic diseases treated by endoscopic retrograde cholangiopancreatography from March 1, 2015 to March 1, 2016 were retrospectively analyzed. The frequency of ERCP operation, the success rate of treatment, the incidence of complications and mortality were compared between the elderly group and the non-elderly group. Results in this study, 236 patients were treated with ERCP lithotomy 267 times. Group A was over 80 years old, group B was aged 60 to 80 years old. Group C was treated with ERCP for 110 times. Group C was a young and middle-aged group aged 18 to 60 years. A total of 137 operations. The rate of improvement of ERCP lithotomy and discharge in group C was 100.00.The success rate of complete lithotomy was 70.6%. The effective rate of ERCP treatment was 75.0%. There was no significant difference in the incidence of postoperative complications such as pancreatitis, cholangitis and hemorrhage. No ERCP related perforation occurred, and a total of 0 ERCP related deaths occurred. Conclusion ERCP is safe and effective in the treatment of choledocholithiasis in the elderly.
