应用等速肌力测试技术对臀肌挛缩症患者臀部肌肉肌力的研究

【摘要】:目的:采用等速肌力测试技术研究臀肌挛缩症患者髋周肌群肌力变化,进一步分析臀肌挛缩症患者并发症的发病机制。方法:2013年6月~2014年7月,选取16例就诊上海市第一康复医院的女性C1型臀肌挛缩症患者作为实验组,同期选取健康女性自愿者16例作为对照组,进行前瞻性研究。应用Contrex等速测试系统在向心60。/S的角速度下分别测定两组受试者髋关节屈伸、旋转、展收3个功能轴肌群肌力的峰力矩、相对峰力矩、屈伸肌群峰力矩比值,分别采用独立样本t检验比较这些指标与正常人的差异。结果:与正常人相比,臀肌挛缩症患者髋关节屈曲、外展、外旋肌群峰力矩(68.31±12.60、65.20±12.20、0.22±0.04)大于正常人髋关节屈曲、外展、外旋肌群峰力矩(64.59±16.17、60.17±12.34、0.20±0.04),但其伸肌肌群峰力矩明显小于正常人,差异有统计学意义(P0.05)。此外,患者臀肌挛缩症患者屈伸肌群峰力矩比(1.23±0.35)明显大于正常人(0.96±0.28),差异有统计学意义(P0.05)。结论:臀肌挛缩症患者髋周肌力失衡,其屈曲、外展、外旋肌群肌肉力量大于正常人,但其伸肌肌力小于正常人。
[Abstract]:Objective: to study the changes of muscle strength of perihip muscles in patients with gluteal muscle contracture by isokinetic muscle strength test, and to further analyze the pathogenesis of complications in patients with gluteal muscle contracture. Methods: from June 2013 to July 2014, 16 female patients with type C1 gluteal muscle contracture were selected as experimental group and 16 healthy female volunteers as control group. Contrex isokinetic testing system was used to measure the peak torque, relative peak moment and the ratio of peak torque of flexion and extensor muscles of hip joint in two groups at the angular velocity of 60 / S, respectively. T-test was used to compare the difference between these indexes and normal subjects. Results: compared with normal subjects, the torque of hip joint flexion, abduction and extrochanter muscle group in gluteal muscle contracture patients (68.31 卤12.60,65.20 卤12.20 卤0.22 卤0.04) was higher than that in normal subjects (64.59 卤16.1760.17 卤12.340.20 卤0.04), but the peak torque of extensor muscle group was significantly lower than that of normal subjects. The difference was statistically significant (P0.05). In addition, the peak torque ratio of flexion and extensor muscle in patients with gluteus contracture was significantly higher than that in normal controls (1.23 卤0.35), the difference was statistically significant (P0.05). Conclusion: in gluteal muscle contracture patients, the muscle strength of the group of flexion, abduction and extrochanter is larger than that of the normal group, but the muscle strength of extensor muscle is smaller than that of the normal group.
