降钙素原诊断成人骨关节感染的Meta分析

【摘要】：背景:骨关节感染因骨特有的解剖生理特征,其治疗成功率仍不理想。成功治疗的关键在于早期诊断,因此寻求一个敏感度与特异度均较高的生物学标志物十分必要。目的:系统评价降钙素原对成人骨关节感染的诊断价值。方法:计算机检索1990年至2017年6月Pubmed、Embase、The Cochrane library及中国知网、中国生物医学文献数据库、万方数据库发表的文献。由2位评价员按照预先设定的纳入与排除标准独立筛选文献、提取资料,利用诊断试验质量评价工具2评价纳入研究的偏倚风险后,采用Stata 14.0软件进行Meta分析,计算合并敏感度、特异度、阳性似然比、阴性似然比、诊断比值比,绘制受试者工作特征曲线并计算曲线下面积。结果与结论:(1)共纳入10篇文献,包含893例患者;(2)降钙素原诊断成人骨关节炎的合并敏感度、特异度、阳性似然比、阴性似然比、诊断比值比分别为0.82(95%CI=0.72-0.89)、0.89(95%CI=0.82-0.93)、7.20(95%CI=4.6-11.3)、0.20(95%CI=0.13-0.32)、35(95%CI=18-71)和0.92(95%CI=0.90-0.94);(3)综上,降钙素原在诊断成人骨关节炎中具有较高的诊断效能。
[Abstract]:Background: the success rate of osteoarticular infection is still not satisfactory because of its unique anatomical and physiological characteristics. The key to successful treatment lies in early diagnosis, so it is necessary to seek a biomarker with high sensitivity and specificity. Objective: to evaluate the diagnostic value of procalcitonin in adult osteoarthrosis. Methods: from 1990 to June 2017, the published articles of Pubmeda Embase library, Chinese Biomedical Literature Database and Wanfang Database were searched by computer. According to the pre-set inclusion and exclusion criteria, two evaluators independently sifted the literature, extracted the data, evaluated the bias risk of the study by using diagnostic test quality evaluation tool 2, and carried out Meta-analysis by Stata 14.0 software. The combined sensitivity, specificity, positive likelihood ratio, negative likelihood ratio and diagnostic ratio were calculated. The operating characteristic curve was drawn and the area under the curve was calculated. Results and conclusion: (1) Ten articles including 893 patients were included; (2) the combined sensitivity, specificity, positive likelihood ratio, negative likelihood ratio of procalcitonin in the diagnosis of adult osteoarthritis were 0.82 (95CI0.72-0.89) 0.89 (95CI0.82-0.93) 7.20 (95CI4.6-11.3) 0.20 (95CI0.13-0.32) 35 (95CI18-71) and 0.92 (95CI18-71) and 0.90-0.94); (3, respectively. Procalcitonin has high diagnostic efficacy in the diagnosis of adult osteoarthritis.
