髋关节置换围手术期深静脉血栓的防治

【摘要】:背景:一项比较物理联合药物与单独物理或药物预防深静脉血栓形成临床效果的荟萃分析显示,联合预防能够降低深静脉血栓形成发生的危险性。目的:分析髋关节置换围手术期深静脉血栓预防的有效措施。方法:采取前瞻性研究,将行髋关节置换后患者350例随机被分为2组,一组应用间歇充气加压装置与标准剂量低分子肝素干预(肝素联合IPC组);另一组只接受标准剂量低分子肝素干预(低分子肝素组)。观察两组病例在围手术期的深静脉血栓发生率。结果与结论:(1)肝素联合IPC组深静脉血栓形成发生率(11%),低分子肝素组深静脉血栓形成发生率(33%);肝素联合IPC组深静脉血栓形成发生率明显低于低分子肝素组(P0.01);(2)两组患者均无症状性肺栓塞发生;(3)结果表明,应用间歇充气加压装置联合标准剂量低分子肝素抗凝防治深静脉血栓较单纯应用标准剂量低分子肝素效果更好。
[Abstract]:Background: a meta-analysis of the clinical efficacy of physical combined drugs versus physical or drug alone in the prevention of deep venous thrombosis has shown that combined prevention can reduce the risk of deep venous thrombosis. Objective: to analyze the effective measures to prevent deep venous thrombosis in perioperative period of hip arthroplasty. Methods: in a prospective study, 350 patients after hip replacement were randomly divided into two groups. One group was treated with intermittent inflatable pressure device and standard dose low molecular weight heparin (heparin combined with IPC group), the other group only received standard dose low molecular weight heparin intervention (low molecular weight heparin group). The incidence of deep venous thrombosis was observed in both groups during perioperative period. Results and conclusion: (1) the incidence of deep venous thrombosis in heparin combined with IPC group (11%), low molecular weight heparin group (33%), and the incidence of deep venous thrombosis in heparin combined with IPC group was significantly lower than that in the low molecular weight heparin group (P0.01); (2). No symptomatic pulmonary embolism occurred in all patients. (3) the results showed that, The effect of intermittent inflatable pressure device combined with standard low molecular weight heparin on the prevention and treatment of deep venous thrombosis was better than that of standard dose of low molecular weight heparin alone.
