老年人脊柱椎体压缩性骨折与骨密度相关性研究

【摘要】:目的:分析50岁以上脊柱椎体压缩性骨折患者及健康查体者的腰椎骨密度,探讨脊柱椎体压缩性骨折与骨密度的相关性,并预测椎体压缩性骨折的骨密度危险性阈值。方法:收集来自2012年1月至2013年12月在山东省文登整骨医院诊疗50岁以上脊柱椎体压缩性骨折患者99名,以及50岁以上的查体患者208名,均进行DR数字摄影及使用DEXA进行骨密度扫描,将测量所得骨密度数值输入SPSS17.0软件进行统计学分析,应用秩和检验、t检验、方差分析以及Person相关性进行分析脊柱椎体压缩性骨折与骨密度的相关性。结果:骨折组腰椎骨密度为0.8217?0.1744 g/cm2,对照组腰椎骨密度为1.0045?0.2076 g/cm2,骨折组骨密度与对照组骨密度差异有显著性(p0.05)。50岁~60岁年龄段骨折组骨密度为0.8745?0.1590 g/cm2,对照组骨密度为1.0284?0.2091 g/cm2;60岁~70岁年龄段骨折组骨密度为0.7932?0.1751 g/cm2,对照组骨密度为0.9922?0.2271 g/cm2;70岁以上年龄段骨折组骨密度为0.7720?0.1604 g/cm2,对照组骨密度为0.9080?0.1881 g/cm2。这3个年龄段骨折组骨密度与对照组骨密度均有显著性差异(p0.05)。Person相关性分析椎体压缩性骨折与骨密度相关性pearon correlation为-2.38,Sig为0.018(p0.05)差异有显著性。脊柱椎体压缩性骨折的骨密度危险性阈值为0.7241 g/cm2。结论:50岁以上老年人脊柱骨密度随着年龄增大而逐步降低,脊柱椎体压缩性骨折危险性随着骨密度的降低而增大。通过所得危险性阈值可以预测脊柱椎体发生压缩性骨折的危险性。
[Abstract]:Objective: to analyze the bone mineral density (BMD) of lumbar vertebrae in patients with vertebral compression fracture over 50 years old and healthy examiner, to explore the correlation between vertebral compression fracture and bone mineral density, and to predict the risk threshold of bone mineral density (BMD) of vertebral compression fracture. Methods: from January 2012 to December 2013, 99 patients with vertebral compression fracture over 50 years old and 208 patients over 50 years old were collected from Wendeng whole Bone Hospital of Shandong Province. Dr digital photography was performed and bone mineral density was scanned by DEXA. The measured bone mineral density was input into SPSS17.0 software for statistical analysis, and rank sum test was used to test the bone mineral density (BMD). Variance analysis and Person correlation were used to analyze the correlation between vertebral compression fracture and bone mineral density. Results: the BMD of lumbar vertebrae in fracture group was 0.82170.1744 g / cm ~ 2, and that in control group was 1.0045 ~ 0.2076 g / cm ~ (-2). There was a significant difference between fracture group and control group (p0.05). The BMD of fracture group was 0.8745 ~ 0.1590 g / cm ~ (-2), and that of control group was 1.02844 ~ 0.2091 g / cm ~ (-2). Bone mineral density (BMD) was 0.7932 / 0.1751 g / cm ~ (-2) in the fracture group of 70 years old, 0.9922 ~ 0.2271 g / cm ~ (-2) in the control group, 0.7720 ~ 0.1604 g / cm ~ (-2) in the control group and 0.9080 / 0.1881 g / cm ~ (-2) in the control group. Correlation analysis of bone mineral density (BMD) between the three age group and the control group (p0. 05). The correlation between vertebral compression fracture and BMD was-2. 38 Sig = 0.018 (p0. 05). The BMD risk threshold for vertebral compression fractures is 0.7241 g / cm ~ 2. Conclusion the bone mineral density (BMD) of spinal column decreases with age and the risk of vertebral compression fracture increases with the decrease of BMD. The risk threshold can be used to predict the risk of vertebral compression fracture.
