磁共振动态增强及波谱成像对骨肉瘤的鉴别诊断价值

【摘要】:目的探讨磁共振动态增强(DCE)曲线类型及氢质子波谱成像(~1H-MRS)对原发性骨肉瘤的应用价值。资料与方法前瞻性纳入41例符合研究标准的骨肉瘤患者,分别行DCE及~1H-MRS扫描,根据其病理结果分为高度和低度恶性骨肉瘤,利用分析软件得到时间-信号强度曲线(TIC)并进行分类;对其中18例~1H-MRS谱线稳定的骨肉瘤患者进行分析并得到Cho及Cho/Cr值,分别进行统计分析。结果高度恶性骨肉瘤中TIC Ⅰ型、Ⅱ型曲线分别有20例、10例,低度恶性骨肉瘤TIC Ⅰ型、Ⅱ型、Ⅲ型曲线分别有2例、5例、4例,高、低度恶性骨肉瘤之间TIC类型分布差异有统计学意义(χ~2=10.876,P0.05)。高、低度恶性骨肉瘤的Cho/Cr值差异有统计学意义(Z=-2.445,P0.05)。以Cho/Cr作为诊断指标,最佳诊断界值为2.3。结论 TIC类型、Cho/Cr值对于评价骨肉瘤的恶性程度有一定的参考价值。
[Abstract]:Objective to investigate the type of dynamic enhanced (DCE) curve and the value of hydrogen proton spectrum imaging (~ 1H-MRS) in primary osteosarcoma. Materials and methods 41 patients with osteosarcoma who met the study criteria were included prospectively. DCE and ~ 1H-MRS scans were performed respectively. According to the pathological results, they were divided into high and low grade malignant osteosarcoma. The time-signal intensity curves (TIC) were obtained and classified by software, and the Cho and Cho/Cr values of 18 osteosarcoma patients with stable spectral line from 1H-MRS were analyzed and analyzed statistically.
