前交叉韧带重建肌腱移植物内预置高强度缝线预防蠕变的实验研究

【摘要】:目的探讨前交叉韧带(anterior cruciate ligament,ACL)重建术肌腱移植物预置高强度线防治术后移植物蠕变松弛的可行性。方法选取成年猪双侧跟腱制备ACL重建移植物标本36个,随机分为A、B、C、D4组,每组9个标本;其中A、C组为实验组,B、D组为对照组。A、C组肌腱内预置1根双股Ultrabraid高强度2号缝线。A、B组及C、D组分别进行1 000次及3 000次循环负荷试验,观察并测量各组移植物于100、500、1 000、2 000、3 000次循环时的松弛度,并测量C、D组的失效载荷。结果 A组各循环次数下移植物松弛度均小于B组,100、500、1 000次循环下两组松弛度比较差异有统计学意义(P0.05);C组各循环次数下移植物松弛度均小于D组,差异均有统计学意义(P0.05)。且C组失效载荷显著高于D组,差异有统计学意义(t=4.816,P=0.001)。结论 ACL重建术中肌腱移植物预置高强度缝线,对防止移植物松弛具有重要作用。
[Abstract]:Objective to investigate the feasibility of pre-implantation of high strength line in tendon graft reconstruction of anterior cruciate ligament (anterior cruciate ligament,ACL) for the prevention and treatment of graft creep relaxation after operation. Methods Thirty-six ACL reconstructed grafts were prepared from bilateral Achilles tendons of adult pigs and were randomly divided into 4 groups with 9 specimens in each group. Among them, group A (C) was used as experimental group, group B (D) as control group, group A (group C) was prepositioned with a double-stranded Ultrabraid high-strength suture (No. 2) in tendon. Group B and group C (group C) were subjected to 1 000 and 3 000 circulatory load tests, respectively. The relaxation of the grafts in each group was observed and measured during 3000 cycles, and the failure load of group C ~ (2 +) was measured. Results the relaxation of grafts in group A was lower than that in group B (P 0.05), and there was a significant difference between the two groups (P0.05). The failure load of group C was significantly higher than that of group D (t = 4.816). Conclusion Preplacement of high strength suture for tendon graft in ACL reconstruction is important to prevent graft relaxation.
