平乐手法与改良DHS术式结合治疗粗隆间骨折的临床研究

【摘要】：目的：评估平乐正骨手法结合DHS治疗老年骨质疏松性股骨粗隆间骨折的综合疗效,为该术式的临床推广提供客观临床依据,同时为中医正骨手法在大关节骨折的应用探索提供经验。方法收集在广州中医药大学附属深圳平乐骨伤科医院老年骨科住院部住院且符合准入条件的老年性股骨粗隆间骨折患者共40例。随机分为2组：实验组：采用平乐手法与改良DHS术式结合治疗,共20例；对照组：采用传统DHS术式治疗,共20例；观察记录40例患者手术时间、出血量、切口长度、髋关节Harris评分、膝关节Lyshonlm评分。应用SPSS19.0软件分析数据。结果实验组与对照组在受伤前Barthel指数、骨折分型、性别及年龄方面差异分布上无统计学意义；实验组与对照组在手术时间、切口长度及术中出血量分别为70.35±14.80、9.15±1.08、220.00+95.14与101.50±26.36、12.00±1.48、344.00+139.10,P0.05,具有统计学意义；实验组与对照组在手术总时间、髋关节Harris评分、膝关节Lyshonlm评分、骨折愈合情况及Garden对线指数之间的比较,P值0.05,差异无统计学意义。结论实验组与对照组在治疗老年性股骨粗隆间骨折均取得了良好的疗效；实验组与对照组相比,切口暴露时间明显缩短,出血量明显减少；中医正骨手法在大关节手术中也能起到重要作用。
[Abstract]:Objective: to evaluate the comprehensive effect of Pingle Zhenggu manipulation combined with DHS in the treatment of senile osteoporotic femoral intertrochanteric fracture and to provide objective clinical basis for its clinical popularization. At the same time, it provides experience for the application of orthopaedic manipulation in the fracture of large joint. Methods A total of 40 cases of senile intertrochanteric fracture of femur in Shenzhen Pingle Orthopaedics Hospital affiliated to Guangzhou University of traditional Chinese Medicine were collected. Two groups were randomly divided into two groups: the experimental group (n = 20) was treated with pingle manipulation combined with modified DHS operation (n = 20), the control group (n = 20) was treated with traditional DHS operation (n = 20), the operation time, blood loss, incision length of 40 patients were observed and recorded. Harris score of hip joint and Lyshonlm score of knee joint. SPSS19.0 software is used to analyze the data. Results there was no significant difference in Barthel index, fracture classification, sex and age between the experimental group and the control group before injury, and the time of operation between the experimental group and the control group. The length of incision and the amount of intraoperative bleeding were 70.35 卤14.80,9.15 卤1.08220.0095.14 and 101.50 卤26.36 卤12.00 卤1.48344.00 139.10m P0.05respectively. There was no significant difference between fracture healing and Garden linear index (P = 0.05). Conclusion both the experimental group and the control group have achieved good results in the treatment of senile intertrochanteric fracture of femur, the time of incision exposure and the amount of blood loss in the experimental group are significantly shorter than those in the control group. Traditional Chinese medicine bone manipulation can also play an important role in major joint surgery.
