采用不同杂交技术行冠脉血运重建术式的探讨

【摘要】:半个多世纪以来,冠状动脉血运重建技术已大大发展。自首次应用体外循环下冠脉旁路移植术对冠脉多支病变进行治疗,在此以后,CABG得到了飞速的发展,不停跳下全胸骨正中切口,纵劈胸骨的CABG;90年代开展的减少患者痛苦的微创小切口CABG以及机器人辅助下全腔镜CABG;经皮冠状动脉介入技术(PCI)从上世纪八十年代至今已发展到金属裸支架(BMS)、药物涂层支架(DES)和可吸收支架,至今技术已经很成熟;1996[1]年Angelini等首次提出了冠状动脉杂交血运重建技术(HCR),此术式包含CABG及PCI,并将两者优点结合到一起,以期达到更加满意的治疗效果。现就杂交冠脉血运重建技术的定义、手术策略及患者选择、抗凝策略,现今的问题以及与杂交冠脉杂交技术密切相关的MIDCAB及机器人辅助下CABG的选择、新进展等做一综述。
[Abstract]:For more than half a century, coronary artery revascularization technology has been greatly developed. Since the first treatment of coronary artery disease with coronary artery bypass grafting under cardiopulmonary bypass (CPB), CABG has developed rapidly, beating CABG; through the median incision of the whole sternum and cutting the sternum longitudinally. Minimally invasive incision CABG and robot-assisted full-endoscopic CABG; developed in the 1990s Percutaneous coronary intervention (PCI) has been developed into metal bare stent (BMS), drug-coated stent (DES) and absorbable stent since 1980s. In 1996 [1], Angelini et al proposed the coronary artery hybrid revascularization technique (HCR),) for the first time, which included CABG and PCI, and combined the advantages of the two techniques in order to achieve a more satisfactory therapeutic effect. In this paper, the definition of hybrid coronary artery revascularization technique, surgical strategy and patient selection, anticoagulant strategy, current problems, selection of MIDCAB and robot-assisted CABG, which are closely related to hybrid coronary artery hybridization, are reviewed.
