切开复位内固定术治疗跖骨基底部粉碎性损伤的疗效

【摘要】：目的探讨切开复位内固定术治疗跖骨基底部粉碎性损伤(Lisfranc损伤)的临床疗效。方法选取2012年3月至2014年7月复旦大学附属中山医院青浦分院骨科住院治疗的65例Lisfranc损伤患者作为研究对象,按照手术方式的不同分为观察组和对照组,其中观察组患者36例接受切开复位内固定术治疗,对照组患者29例接受关节融合术治疗。比较两组患者的手术时间、术中出血量、术后住院时间、AOFAS足评分以及术后并发症发生情况。结果观察组患者术中出血量低于对照组,手术时间及术后住院时间高于对照组,差异均有统计学意义(t=-4.689、4.522、3.120,P均0.05);观察组术后的疼痛值得分、功能值得分、力线值得分、总得分以及AOFAS足评分等级均优于对照组,差异均有统计学意义(t=0.547、9.768、12.323、4.745、12.253,P均0.05);观察组患者并发症发生率为22.22%,对照组为24.14%,差异无统计学意义(P0.05)。结论切开复位内固定对Lisfranc损伤患者具有较明显疗效,值得临床推广应用。
[Abstract]:Objective to investigate the clinical effect of open reduction and internal fixation in the treatment of comminuted injury of metatarsal base (Lisfranc). Methods from March 2012 to July 2014, 65 patients with Lisfranc injury in the Department of Orthopaedics, Qingpu Branch Hospital, affiliated Hospital of Fudan University were selected as the subjects, and were divided into observation group and control group according to different surgical methods. 36 patients in the observation group were treated with open reduction and internal fixation, and 29 patients in the control group were treated with arthrodesis. The operative time, intraoperative bleeding, postoperative hospital stay, AOFAS foot score and postoperative complications were compared between the two groups. Results the amount of bleeding during operation in the observation group was lower than that in the control group, and the operative time and hospitalization time in the observation group were significantly higher than those in the control group (t = 4.689,4.522,3.120, P < 0.05). The postoperative pain score, functional score, force line score, total score and AOFAS foot score in the observation group were significantly better than those in the control group (t = 0.547, 9.768, 12.323, 4.745, 12.253, P 0.05). The incidence of complications was 22.22% in the observation group and 24.14% in the control group. There was no significant difference between the two groups (P0.05). Conclusion Open reduction and internal fixation is effective in the treatment of Lisfranc injury, and it is worthy of clinical application.
