太原及周边地区701例上消化道异物的临床特点及治疗分析

【摘要】：目的:探讨上消化道异物的临床特点,并对本地区常见上消化道异物内镜下治疗方法予以总结,以期为临床诊断及治疗提供参考依据。方法:回顾性分析山西医科大学第一医院2010年1月~2015年6月因上消化道异物就诊患者的病例资料,对其临床特点进行分析,并对本地区常见上消化道异物内镜下治疗方法予以总结。应用SPSS19.0统计学软件进行分析,计数资料采用X2检验,P0.05为差异有统计学意义。结果:1.701例患者中共发现异物675个。异物类型以食源性异物最多,其中枣核最常见(80%,χ~2=244.56,P0.05)。2.食管为最常见的嵌顿的部位(90.8%,χ~2=448.81,P0.05),以食管上段最常见(85.8%,χ~2=483.56,P0.05)。3.合并相关上消化道疾病中以食管癌及食管狭窄者最常见(64.5%)。4.鼠齿钳及网篮为治疗上消化道异物最常用的辅助器械,斑马导丝为胃石症碎石治疗最常用的辅助器械。5.胃镜治疗上消化道异物成功率高(近100%),无操作相关的严重并发症发生。结论:1.枣核是太原及周边地区最常见的异物类型。2.上消化道异物嵌顿是临床常见急症,绝大多数患者需内镜下治疗。3.内镜是治疗上消化道异物安全有效的方法。
[Abstract]:Aim: to investigate the clinical characteristics of foreign bodies in upper gastrointestinal tract and summarize the endoscopic treatment of foreign bodies in upper digestive tract in order to provide reference for clinical diagnosis and treatment. Methods: the clinical characteristics of patients with foreign bodies in upper digestive tract from January 2010 to June 2015 in the first Hospital of Shanxi Medical University were analyzed retrospectively. The methods of endoscopic treatment of foreign bodies in upper gastrointestinal tract in this area were summarized. SPSS19.0 statistical software was used for analysis, counting data using X2 test, P0.05 was statistically significant. Results: a total of 675 foreign bodies were found in 1.701 patients. Among them, jujube nucleus was the most common (80%, 蠂 ~ 2, 244.56, P0.05). The esophagus was the most common site of incarceration (90.8%, 蠂 ~ 2 / 448.81, P0.05), and the upper segment of esophagus was the most common (85.8%, 蠂 ~ 2 / 483.56, P0.05). Esophageal cancer and esophageal stricture were the most common diseases associated with upper gastrointestinal diseases (64.5%). The most commonly used aids for the treatment of foreign bodies in the upper digestive tract are rat teeth forceps and net baskets, and zebra guide wire is the most commonly used auxiliary instrument for the treatment of gastrolithotripsy. 5. The success rate of gastroscopy in the treatment of foreign bodies in upper digestive tract was high (nearly 100%), and there were no serious complications associated with operation. Conclusions: 1. Jujube kernel is the most common foreign body type in Taiyuan and its surrounding areas. 2. Incarceration of foreign bodies in upper gastrointestinal tract is a common clinical emergency, and most patients need to be treated by endoscopy. 3. Endoscopy is a safe and effective method for the treatment of foreign bodies in upper digestive tract.
