高频超声检测软组织内异物的临床价值分析

【摘要】：目的探讨高频超声检测软组织内异物的临床价值。方法选择2016年1—6月江苏大学附属上海第八医院普外科收治的215例疑有软组织异物残留的入院患者,术前均行高频超声检测,以实时超声引导下手术探查结果为金标准,分析高频超声检测软组织内异物的临床价值。结果 215例患者手术共探查307处软组织,实际有异物数275个,实际无异物数32个。术前高频超声检测262个异物阳性,误诊3个,45个异物阴性,漏诊16个,其灵敏度为94.2%,特异度为90.6%,漏诊率为5.8%,误诊率为9.4%,正确率为93.8%,阳性似然比为10.046,阴性似然比为0.064,约登指数为0.848。结论高频超声廉价、安全、高效,其检测软组织内异物的正确率高,具有重要的临床价值,可指导手术实施。
[Abstract]:Objective to evaluate the clinical value of high-frequency ultrasound in detecting foreign bodies in soft tissue. Methods 215 patients with suspected residual soft tissue foreign bodies were selected from January to June 2016 in the General surgery Department of Shanghai eighth Hospital affiliated to Jiangsu University. All patients were examined by high frequency ultrasound before operation. The results of real-time ultrasound guided exploration were used as the gold standard. To analyze the clinical value of high frequency ultrasound in detecting foreign body in soft tissue. Results 307 soft tissues were explored in 215 cases, 275 foreign bodies and 32 foreign bodies were found. Of 262 foreign bodies detected by high-frequency ultrasound before operation, 3 were misdiagnosed, 45 were negative, and 16 were missed. The sensitivity, specificity, missed diagnosis and misdiagnosis were 94.2%, 90.6%, 5.8% and 9.4%, respectively. The correct rate is 93.8%, the positive likelihood ratio is 10.046, the negative likelihood ratio is 0.064, and the Joden index is 0.848. Conclusion High frequency ultrasound is cheap, safe and efficient. It has high accuracy in detecting foreign bodies in soft tissue. It has important clinical value and can guide the operation.
