半髋关节置换术与内固定术治疗股骨转子间骨折效果观察

【摘要】：目的对比观察半髋关节置换术与内固定术治疗股骨转子间骨折的效果。方法选择股骨转子间骨折患者151例,随机分为A组(51例)、B组(48例)、C组(52例),分别采用半髋关节置换术、股骨近端髓内钉固定术、动力髋螺钉固定术进行治疗。记录三组手术时间、出血量、下床时间、住院时间;术后12个月采用Harris评分评价髋关节功能;记录术后12个月内并发症及死亡情况。结果手术时间、术中出血量、下床时间A组B组C组,两两比较差异有统计学意义(P均0.05);三组住院时间比较差异无统计学意义(P0.05)。A组优良率高于B、C组(P均0.05),B、C组比较差异无统计学意义(P0.05)。A、B组并发症发生率低于C组(P0.05),A、B组比较差异无统计学意义(P0.05)。A组术后12个月病死率低于B、C组(P0.05),B、C组比较差异无统计学意义(P均0.05)。结论与内固定术治疗比较,半髋关节置换术治疗股骨转子间骨折效果更佳,病死率及并发症发生率更低。
[Abstract]:Objective to compare the effect of half hip arthroplasty and internal fixation in the treatment of intertrochanteric fracture. Methods A total of 51 patients with intertrochanteric fracture were randomly divided into group A (51 patients with), B and 48 patients with), C, 52 patients). The patients were treated with half hip arthroplasty, proximal femoral intramedullary nail fixation and dynamic hip screw fixation respectively. The time of operation, the amount of bleeding, the time of getting out of bed and the length of hospitalization were recorded, the hip function was evaluated by Harris score at 12 months after operation, and the complications and death were recorded within 12 months after operation. Results the time of operation, the amount of bleeding during operation and the time of getting out of bed in group A, group B and group C were significantly different (P < 0.05), and there was significant difference between the two groups (P < 0.01). There was no significant difference in hospitalization time among the three groups (P0.05,). A group was higher than B, C group (all P 0.05), B, C group had no significant difference (P0.05). A,). The incidence of complications in group B was lower than that in group C (P0.05). There was no significant difference in mortality between group A and group B (P0.05). A group was lower than that in group B and group C at 12 months after operation) (P0.05), B,). There was no significant difference in group C (P 0.05). Conclusion compared with internal fixation, half hip arthroplasty is more effective in the treatment of femoral intertrochanteric fractures, with lower mortality and lower incidence of complications.
