桃红四物汤合五味消毒饮加减治疗全膝关节置换术后炎症反应疗效的临床研究

【摘要】：目的：研究桃红四物汤合五味消毒饮加减治疗全膝关节置换术后炎症反应的临床疗效,有利于为临床提供一种疗效更佳的综合疗法。方法：选择符合标准的行初次单侧全膝关节置换术患者70例,用统计软件随机分为2组：实验组(35例)和对照组(35例)。对照组术后予常规治疗,实验组在对照组常规治疗基础上加服桃红四物汤合五味消毒饮加减(每天1剂,连服14天)。分别于术前1天、术后3、7、14天对比两组患者血清TNF-α、IL-6、CRP、膝关节肿胀程度、视觉模拟疼痛评分及关节活动度。结果：1.血清TNF-α:组间比较：术前两组差异无统计学意义(P0.05)；术后3、7、14天实验组低于对照组,差异均有统计学意义(P0.05)。组内比较：两组均有术后7天低于术后3天、术后14天低于术后7天,差异均有统计学意义(P0.05)。2.血清IL-6：组间比较：术前两组差异无统计学意义(P0.05)；术后3、7、14天实验组低于对照组,差异均有统计学意义(P0.05)。组内比较：两组均有术后7天低于术后3天、术后14天低于术后7天,差异均有统计学意义(P0.05)。3.血清CRP：组间比较：术前两组差异无统计学意义(P0.05)；术后3、7、14天实验组低于对照组,差异均有统计学意义(P0.05)。组内比较：两组均有术后7天低于术后3天、术后14天低于术后7天,差异均有统计学意义(P0.05)。4.膝关节肿胀程度：组间比较：术前两组差异无统计学意义(P0.05)；术后3、7、14天实验组低于对照组,差异均有统计学意义(P0.05)。组内比较：两组均有术后7天低于术后3天、术后14天低于术后7天,差异均有统计学意义(P0.05)。5.膝关节视觉模拟疼痛评分：组间比较：术前两组差异无统计学意义(P0.05)；术后3、7天实验组低于对照组,差异均有统计学意义(P0.05)。组内比较：两组均有术后7天低于术后3天、术后14天低于术后7天,差异均有统计学意义(P0.05)。6.膝关节活动度：组间比较：术前两组差异无统计学意义(P0.05)；术后3、7天实验组优于对照组,差异均有统计学意义(P0.05)。组内比较：两组均有术后7天优于术后3天、术后14天优于术后7天,差异均有统计学意义(P0.05)。结论：桃红四物汤合五味消毒饮加减可有效治疗全膝关节置换术后炎症反应,有助于机体康复。
[Abstract]:Objective: to study the clinical effect of Taohong Siwu decoction combined with Wuwei disinfecting decoction in the treatment of inflammatory reaction after total knee arthroplasty, which is beneficial to provide a better comprehensive therapy for clinic. Methods: 70 patients undergoing initial unilateral total knee arthroplasty were randomly divided into two groups: experimental group (n = 35) and control group (n = 35). The control group was treated with routine treatment after operation, and the experimental group was treated with Taohong Siwu decoction combined with Wuwei Disinfection decoction on the basis of routine treatment in the control group (1 dose per day for 14 days). The swelling degree of serum TNF- 伪 and IL-6,CRP, knee joint, visual simulated pain score and joint range of motion were compared between the two groups 1 day before operation, 3 days after operation, 7 days after operation and 14 days after operation. Results: 1. Serum TNF- 伪: there was no significant difference between the two groups before operation (P 0.05), and the experimental group was lower than the control group at 3, 7 and 14 days after operation, and the difference was statistically significant (P 0.05). Intra-group comparison: 7 days after operation was lower than 3 days after operation, 14 days after operation was lower than 7 days after operation, the difference was statistically significant (P 0.05). There was no significant difference in serum IL-6: between the two groups before operation (P 0.05), and the experimental group was lower than the control group at 3, 7 and 14 days after operation, and the difference was statistically significant (P 0.05). Knee joint visual analog pain score: there was no significant difference between the two groups before operation (P 0.05), and the experimental group was lower than the control group on the 3rd and 7th day after operation, the difference was statistically significant (P 0.05). Intra-group comparison: 7 days after operation was lower than 3 days after operation, 14 days after operation was lower than 7 days after operation, the difference was statistically significant (P 0.05). Knee joint range of motion: there was no significant difference between the two groups before operation (P 0.05), and the experimental group was superior to the control group on the 3rd and 7th day after operation, and the difference was statistically significant (P 0.05). Intra-group comparison: 7 days after operation was better than 3 days after operation, 14 days after operation was better than 7 days after operation, the difference was statistically significant (P 0.05). Conclusion: Taohong Siwu decoction combined with Wuwei Disinfection decoction can effectively treat the inflammatory reaction after total knee arthroplasty and contribute to the rehabilitation of the body.
