CEA、CA153、TPS、Fer、CYFRA21-1检测在乳腺癌术后转移中的诊断价值

【摘要】：目的探索肿瘤标志物CEA、CA153、TPS、Fer、CYFRA21-1在乳腺癌术后转移中的诊断价值。方法选择2012-2014年在唐山市人民医院治疗的578例原发性乳腺癌患者,随访3年后根据有、无转移分为转移组42例,从未出现转移患者中随机抽取42例作为未转移组。采用化学发光法检测患者CEA、CA153、TPS、Fer、CYFRA21-1水平,比较转移组与未转移组CEA、CA153、TPS、Fer、CYFRA21-1水平差异;采用Pearson相关性检验分析在乳腺癌术后转移患者中TPS、Fer、CYFRA21-1三者的相关性;采用受试者ROC曲线检验各组数值对乳腺癌术后转移的检验效率。结果乳腺癌术后转移组患者血清CEA、CA153、TPS、Fer、CYFRA21-1水平[分别为(6.11±1.87)ng/m L、(26.97±9.52)U/m L、(155.95±22.03)U/L、(173.08±19.87)ng/m L、(18.98±7.33)ng/m L]比未转移组[分别为(5.33±1.85)ng/m L、(23.34±8.30)U/m L、(133.22±19.09)U/L、(142.29±22.59)ng/m L、(6.76±1.59)ng/m L]升高,TPS、Fer、CYFRA21-1水平变化更明显,差异有统计学意义(P0.05);乳腺癌术后转移患者血清TPS、Fer、CYFRA21-1均未见明显相关性;CYFRA21-1的ROC曲线下面积(0.903)Fer(0.849)TPS(0.791),在乳腺癌术后转移的诊断中,CYFRA21-1曲线下面积最大具有较高的诊断价值。结论乳腺癌患者术后血清TPS、Fer、CY-FRA21-1的水平升高与乳腺癌术后发生转移密切相关,CYFRA21-1对于监测乳腺癌术后转移具有较高的价值。
[Abstract]:Objective to explore the diagnostic value of tumor marker CEA,CA153,TPS,Fer,CYFRA21-1 in postoperative metastasis of breast cancer. Pearson correlation test was used to analyze the correlation of TPS,Fer,CYFRA21-1 in patients with postoperative metastasis of breast cancer, and ROC curve was used to test the efficiency of each group in detecting postoperative metastasis of breast cancer. Results the serum CEA,CA153,TPS,Fer,CYFRA21-1 levels in patients with postoperative metastasis of breast cancer were (6.11 卤1.87) ng/m / L, (26.97 卤9.52) U / L, (155.95 卤22.03) U / L, respectively. (173.08 卤19.87) ng/m / L, (18.98 卤7.33) ng/m / L, (5.33 卤1.85) ng/m / L, (23.34 卤8.30) U / L, (133.22 卤19.09) U / L, (142.29 卤22.59) ng/m / L, (6.76 卤1.59) ng/m / L] increased, and the change of TPS,Fer,CYFRA21-1 level was more obvious, the difference was statistically significant (P 0.05). There was no significant correlation between serum TPS,Fer,CYFRA21-1 in patients with postoperative metastasis of breast cancer. The area under ROC curve of CYFRA21-1 (0.903) Fer (0.849) TPS (0.791). In the diagnosis of postoperative metastasis of breast cancer, the area under CYFRA21-1 curve is the most valuable in the diagnosis of postoperative metastasis of breast cancer. Conclusion the increase of serum TPS,Fer,CY-FRA21-1 level in patients with breast cancer is closely related to postoperative metastasis of breast cancer. CYFRA21-1 is of great value in monitoring postoperative metastasis of breast cancer.
