改良缝合法治疗急性闭合性跟腱断裂的疗效分析

【摘要】：目的探讨使用改良缝合法治疗急性闭合性跟腱断裂的临床疗效。方法 2013年12月—2016年12月,采用改良缝合法治疗29例急性闭合性跟腱断裂。男23例,女6例;年龄22~45岁,平均34岁。患者均为运动损伤,其中左侧22例、右侧7例。术前美国足踝外科协会(AOFAS)踝与后足功能评分为(44.6±3.6)分;参照Amer-Lindholm疗效评定标准评价均为差。MRI检查显示跟腱完全断裂。结果手术时间35~62 min,平均46 min;术中出血量7~15 m L,平均10 m L。术后切口均Ⅰ期愈合,无腓肠神经损伤发生。患者均获随访,随访时间5~38个月,平均18.4个月。术后3个月,AOFAS踝与后足功能评分为(93.1±4.3)分,与术前比较差异有统计学意义(t= 49.581,P=0.000)。参照Amer-Lindholm疗效评定标准评价,获优24例、良5例,优良率100%;与术前比较差异有统计学意义(Z= 7.294,P=0.000)。MRI检查示跟腱连续性完整。随访期间均无跟腱再断裂发生。结论改良缝合法治疗急性闭合性跟腱断裂具有创伤小、再断裂发生率低、腓肠神经损伤风险低及功能恢复较好等优点。
[Abstract]:Objective to investigate the clinical effect of modified suture in the treatment of acute closed Achilles tendon rupture. Methods from December 2013 to December 2016, 29 cases of acute closed Achilles tendon rupture were treated with modified suture. There were 23 males and 6 females with an average age of 34 years. All the patients had motor injury, including 22 cases on the left side and 7 cases on the right side. The score of ankle and posterior foot function of the American Association of foot and ankle surgery was (44.6 卤3.6) before operation, and the evaluation was poor according to the evaluation standard of Amer-Lindholm. MRI showed complete rupture of Achilles tendon. Results the operation time was 35 鈮，
