人工关节置换术与内固定术治疗老年股骨粗隆间骨折的Meta分析

【摘要】：目的通过Meta分析比较人工关节置换术与内固定术在治疗股骨粗隆间骨折的疗效。方法检索1979年至2011年关于股骨粗隆间骨折采用人工关节置换术与内固定术治疗对照研究的中、英文文献,采用循证医学Meta分析对患者术后死亡率、术后内科并发症发生率、术后髋部畸形率、术后内固定物松动率及术后髋关节功能评分(Harri ship score)进行综合分析。结果 Meta分析发现,与内固定手术相比,人工关节置换术在治疗股骨粗隆间骨折术后的髋关节功能评分中有明显优势(P0.01),而人工关节置换组的术后死亡率却比内固定组高。在术后髋部畸形率,术后内科并发症发生率,术后内固定物松动率方面,二者无统计学差异。结论在治疗股骨粗隆间骨折的手术方式选择中,人工关节置换术较内固定术在术后髋关节功能评分方面优势明显。
[Abstract]:Objective to compare the efficacy of artificial joint replacement and internal fixation in the treatment of intertrochanteric fracture of femur by Meta analysis. Methods from 1979 to 2011, the Chinese and English literatures on the treatment of intertrochanteric fracture of femur with artificial joint replacement and internal fixation were searched. Evidence-based medicine (Meta) was used to analyze the postoperative mortality, the incidence of postoperative medical complications, the postoperative hip deformity rate, the loosening rate of postoperative internal fixation and the postoperative hip joint function score (Harri ship score). Results Meta analysis showed that compared with internal fixation, artificial joint replacement had obvious advantages in the score of hip joint function after treatment of intertrochanteric fracture of femur (P01), but the postoperative mortality of artificial joint replacement group was higher than that of internal fixation group. There was no significant difference in the rate of hip deformity, the incidence of postoperative medical complications and the loosening rate of internal fixation between the two groups. Conclusion artificial joint replacement is superior to internal fixation in the functional score of hip joint in the choice of surgical methods for the treatment of intertrochanteric fracture of femur.
