肩袖修复重建的系列基础研究

目的肩袖损伤是肩部最常见的病变之一,常需要手术治疗。近年来,虽然肩袖修补术有着长足进步,术后的再撕裂率和并发症的发生率仍然相当高[1,2],肩袖与骨的愈合能力较差是造成术后再撕裂率偏高的主要原因。肩袖损伤后,腱-骨止点的结构破坏,修补后的肌腱与骨的愈合只能形成纤维疤痕组织,从而容易再次损伤[3,4]。本实验为了探究腱-骨愈合与力学刺激、内环境和生物材料的关系,设计了一系列针对肩袖损伤修补及重建的基础性研究。将原位肩袖组织进行穿骨隧道固定,探究隧道内植入式肩袖修复方式中肌腱在骨隧道中的腱-骨愈合情况;并且利用双极电纺纤维膜在腱-骨愈合处诱导腱-骨愈合中胶原纤维增生和骨长入,了解双极纳米纤维膜在肌腱与骨的愈合中的作用;在巨大肩袖损伤的治疗中,利用自体肌腱桥接巨大肩袖损伤中肌腱与骨的缺损,与补片桥接法做对比,评估自体肌腱桥接修复治疗巨大肩袖损伤的临床可行性,通过观察移植肌腱在骨隧道中的重塑,综合了解肩袖修补和重建中的腱-骨愈合情况,寻找促进愈合的手术方式及新型生物材料方法加强腱-骨愈合的微结构再生。方法1.取新西兰白兔建立肩袖损伤模型,在足印区建立...
