采用皮质骨椎弓根螺钉TLIF与传统椎弓根螺钉TLIF的疗效比较

背景:腰椎间孔椎间融合术(TLIF)是一种常见的腰椎融合手术,广泛应用于腰椎退行性椎间盘疾病的治疗。在椎间融合手术中使用传统椎弓根螺钉(PS)进行腰椎固定有几个优点,但同时也存在着一些缺点,如侵犯上关节面和肌肉损伤的风险。最近,人们发明了一种新的椎弓根螺钉植入轨迹,称为皮质骨轨迹(CBT),它可以在不存在传统椎弓根螺钉缺陷的情况下进行插入。然而,使用CBT的TLIF与使用PS TLIF临床疗效尚未得到充分评估。目的:比较皮质骨螺钉轨道(CBT)和传统椎弓根螺钉(PS)技术在腰椎间孔椎间融合术(TLIF)手术中的临床疗效。研究设计:这是回顾性观察研究。方法与材料:收集东南大学附属中大医院脊柱中心单间隙TLIF手术合格患者71例,分为A组(45例),采用PS,B组(26例),采用CBT螺钉。使用X线片和计算机断层扫描(CT)在1年时评估融合状态。通过背部和腿部疼痛的VAS评分、Oswetry残疾指数(ODI)评分和日本骨科协会(JOA)评分评估临床结果。同时记录两组患者的手术时间、失血量,切口长度和术后住院时间,并进行比较。两组患者术后随访1年。采用不同的统计方法进行数据分析,P值<...
