音频分析法评价推拿治疗单纯性鼾症临床疗效的研究

【摘要】:目的: 在第一部分实验中,为了寻求一种可替代多导睡眠观测的经济可行而又具备可以提供客观数据的观察方法,测试音频采集结合计算机软件音频分析方法对单纯性鼾症患者鼾声数据监测的可行性;同时观测单纯性鼾症患者鼾症夜间睡眠期鼾症行为特点,以为下一步实验提供客观可行的观察切入点。 在第二部分实验中,根据第一部分实验的研究结果,通过随机对照临床试验,并拟采用音频分析方法,观察推拿疗法对单纯性鼾症患者夜间睡眠期鼾症行为的影响。 方法: 在第一部分实验中,通过对10例单纯性鼾症患者进行为期2周的睡眠期鼾声音频采集,得到数字化音频文件;通过计算机软件分析音频波形,得到鼾声强度、打鼾次数等数据,将数据录入记录表;测试音频采集结合计算机软件音频分析方法对单纯性鼾症患者鼾声数据监测的可行性;利用统计学分析方法,分析基于音频分析得出的单纯性鼾症患者鼾症夜间睡眠期鼾症行为特点,及确定实际可测量数据。 在第二部分实验中,根据第一部分实验的研究结果,进行下一阶段的临床随机对照实验。对20例单纯性鼾症患者按随机数字分组法分为观察组和对照组;观察组予以推拿手法治疗,对照组按辨证采用中药方剂治疗;两组患者均在治疗前、治疗过程的2周和治疗后2周中采用录音工具对其进行睡眠期录音;完成全过程后回收录音数据,利用计算机软件对其进行音频处理和分析,将数据录入记录表格。通过统计学计算方法,分析推拿对单纯性鼾症的近期、中期及远期疗效。 成果: 在第一部分实验中,证实了音频分析用于对单纯性鼾症患者观测中的可行性,对已发现的不足提出了初步解决方法;同时确定对单纯性鼾症观测时间为入睡后第2至第3小时,前期及长期观察日期为3天,并以鼾声强度和打鼾次数作为下一步实验中观察点。 在第二部分实验中,观察组患者鼾声强度降低及打鼾次数减少优于对照组,且P0.05,差异具统计学意义;提示推拿治疗单纯性鼾症有较好的近期疗效。在中远期疗效观察中,两组患者的鼾声强度及打鼾次数均有明显回升,组间比较,P0.05,差异无统计学意义,提示两种疗法对于单纯性鼾症的中远期疗效均不理想。 结论: 音频分析可作为单纯性鼾症临床研究的可行性手段。推拿对单纯性鼾症有较好的近期疗效,但中远期疗效不理想;应当结合中药治疗、自我按摩保健和单纯性鼾症健康教育(戒烟戒酒、锻炼、减轻体重、侧卧睡眠等),以取得较满意的长期疗效。
[Abstract]:Objective: in the first part of the experiment, in order to find an economical and feasible alternative to polysomnography and to provide objective data, The feasibility of monitoring the snoring data of patients with simple snoring and the characteristics of snoring behavior during nocturnal sleep in patients with simple snoring were studied by using the method of audio acquisition and computer software audio analysis. It provides an objective and feasible entry point for the next experiment. In the second part of the experiment, according to the results of the first part of the experiment, the effects of massage therapy on nocturnal snoring behavior of patients with simple snoring were observed by randomized controlled clinical trial and audio frequency analysis. Results: in the first part of the experiment, the feasibility of audio frequency analysis in the observation of patients with simple snoring was confirmed, and the preliminary solution to the problems found was put forward. At the same time, the observation time of simple snoring was determined to be the 2nd to 3rd hour after falling asleep, the early and long-term observation dates were 3 days, and the snoring intensity and snoring times were taken as the observation points in the next experiment. In the second part of the experiment, the snoring intensity and snoring times in the observation group were lower than those in the control group, and the difference was statistically significant (P 0.05), which suggested that massage had a better short-term effect on simple snoring. In the observation of middle and long term curative effect, the snoring intensity and snoring times of the two groups were obviously increased. The difference between the two groups was not statistically significant (P0.05), which suggested that the two treatments were not ideal for the patients with simple snoring. Conclusion: audio frequency analysis can be used as a feasible method for clinical study of simple snoring. Massage has a good short-term effect on simple snoring, but the medium- and long-term effect is not ideal. It should be combined with traditional Chinese medicine treatment, self-massage health care and simple snoring health education (quit smoking and alcohol, exercise, weight loss, Side-lying sleep, etc.), in order to obtain a more satisfactory long-term effect.
