兔视网膜多波长激光光凝术后光相干断层扫描图像与组织病理学图像的比较

【摘要】:目的: 探讨多波长激光光凝术后视网膜、脉络膜结构的光相干断层扫描(OCT)图像与组织病理学图像变化的相关性。 方法: 有色兔36只,左眼为空白对照组,右眼为实验组,随机分3组,分别用绿、黄、红光激光光凝兔右眼视网膜后极部,光斑反应TsoⅢ级。视网膜激光光凝术后术1h、7d、14d、28d分别对光凝区行OCT和组织病理学检查。 结果: (1)组织病理学检查:三种激光光凝后1h,视网膜脉络膜组织不同程度受到破坏。绿光光凝斑处主要显示视网膜色素上皮(RPE)层色素大部分甚至全部缺失、光感受器细胞层内外节结构严重紊乱,其次为脉络膜毛细血管轻度扩张,外核层核固缩。黄光光凝斑处主要显示脉络膜毛细血管扩张明显、RPE层色素大量缺失、光感受器细胞层内外节结构紊乱,其次是内外核层见核固缩。红光光凝斑处主要是脉络膜大部分血管扩张,其次是RPE层光感受细胞层轻度破坏、内外核层细胞缺失。继后三种激光斑处RPE细胞、Müller细胞及成纤维细胞增生修复光凝区。 (2)OCT影像:三种激光光凝后1h,脉络膜组织的反射均显示欠清,视网膜组织结构显示不清,,视网膜神经上皮层(RNL)显示高反射,全层增厚隆起。绿光、黄光、红光光斑处RNL平均厚度(μm)分别为:265.81±5.39,227.90±6.11,193.76±8.76。绿光光凝斑处RPE层光带变薄甚至消失,黄光光凝斑处RPE层连续性大部分中断,红光光凝斑处RPE层连续性仅少部分中断。光凝后7d,三种激光斑处RNL内出现RPE反射,RNL全层向光凝斑中心牵拉凹陷。绿光、黄光、红光光斑处RNL平均厚度(μm)分别为:72.68±5.99,99.91±4.65,119.73±5.48。随后,RNL内的RPE反射光带不断增厚,遮挡脉络膜组织反射。 结论: 1、激光光凝术后光凝斑OCT图像变化与组织病理学结构的变化具有一致性,可作为激光光凝术后活体观察视网膜及脉络膜组织结构变化的影像工具之一。 2、不同波长的激光作用于眼底的组织学深度不同,激光波长越长,作用于眼底的组织学深度越深。绿光主要作用于RPE层,黄光主要作用于RPE层及脉络膜内层,红光主要作用于脉络膜层。
