【摘要】:目的:1.观察伴与不伴脑出血的高血压患者血清叶酸、Hcy水平差异;探讨血清Hcy、叶酸与高血压脑出血的关系; 2.观察伴与不伴脑出血的高血压患者MTHFR C677T基因型及等位基因频率差异;探讨MTHFR基因多态性与高血压脑出血的关系; 方法:采用病例-对照研究方法,,选择2011年3月至2012年9月就诊于我院心内三科及神经外科的原发性高血压患者382例,根据CT和(或)MRI证实是否有脑出血,分为脑出血组182例和单纯高血压组200例;采用循环酶法检测血浆同型半胱氨酸(Hcy),以Hcy≥10umol/L作为高同型半胱氨酸的诊断标准,高血压伴高Hcy者为“H型”高血压组347例,不伴Hcy升高者为非“H型”高血压组35例;用化学发光法检测血浆中叶酸水平;运用聚合酶链式反应一限制性内切酶片段长度多态性技术(PCR-RFLP),检测MTHFR基因表型。收集一般资料并检测生化指标。 结果:1.高血压脑出血组血浆Hcy水平高于单纯高血压组[(21.56±10.75)umol/l]vs[(17.398±9.26)umol/l(P0.05)],且Hcy随着叶酸水平的降低而增高,二者呈负性相关[(r=-0.43,P0.05)];男性血浆Hcy水平高于女性[(22.18±11.59)umol/l]vs[(15.59±6.20)umol/l(P0.05)];有吸烟、饮酒史患者的血浆Hcy水平高于无吸烟、饮酒史者(P0.05);血浆Hcy与年龄、血脂无明显关系(P>0.05);“H型”高血压患者发生脑出血的比例高于非“H型”高血压者(49.3%比31.4%,P0.05);进行高血压分级后,高血压病3级患者中脑出血组“H型”高血压比例显著高于无脑出血组(96.4比88.5,P0.05),而在高血压病2级中二者比较无差别(P0.05); 2.单纯高血压组与高血压性脑出血组之间CC、CT、TT基因型频率及等位基因频率比较无显著性差异(P>0.05);二组中TT基因型及CT型血浆Hcy水平均显著高于CC型(P0.05),TT基因型和CT基因型之间差异无统计学意义,而叶酸水平显著低于CC基因型; TT基因型中男性所占比例显著高于CT型,CT基因型高于CC基因型;TT和CT基因型患者的体重指数显著高于CC型患者(P0.05)。 3.多因素Logistic回归分析显示:影响高血压患者脑出血发生的独立危险因素有:Hcy(OR=1.08,P0.01)、收缩压(SBP,OR=1.04; P0.01)和舒张压(DBP,OR=1.05;P0.01)。 结论:1.“H型”高血压患者存在更高的脑出血发生风险,血浆Hcy可能与高血压在脑出血的发生上存在交互促进作用; 2.MTHFR基因多态性可能是血浆Hcy升高的易感基因,但与高血压性脑出血的发生可能无相关性。
[Abstract]:Objective 1. To observe the difference of serum folate Hcy levels between hypertensive patients with and without intracerebral hemorrhage, and to explore the relationship between serum Hcyand folic acid and hypertensive intracerebral hemorrhage. 2. To observe the difference of MTHFR C677T genotype and allele frequency between hypertensive patients with and without cerebral hemorrhage, to investigate the relationship between MTHFR gene polymorphism and hypertensive intracerebral hemorrhage. Methods: a case-control study was conducted in 382 patients with essential hypertension who were admitted to our hospital from March 2011 to September 2012 in three departments of cardiology and neurosurgery.
