血清网膜素-1水平与冠状动脉粥样硬化性心脏病的相关性研究

【摘要】:目的:探究血清网膜素-1与冠心病的发生、冠脉病变程度及斑块稳定性的关系。方法:选取2013年10月至2014年3月因“胸痛、心悸、胸闷”收入南华大学附属第一医院心内科住院的患者110例,并行冠状动脉造影(Coronary angiography,CAG)检查,其中冠心病(Coronary heart disease)患者90例及对照组20例。记录110例研究对象的性别、年龄、吸烟史、体重、身高、收缩压(SBP)及舒张压(DBP)。将90例CHD患者分为急性心肌梗死(AMI)组28例、稳定型心绞痛(SAP)组28例及不稳定型心绞痛(UAP)组34例;根据冠脉造影结果将患者分为单支病变组(36例)、双支病变组(32例)、多支病变组(22例)。入院第二天清晨空腹(禁食、禁饮8h以上)抽血检测110例研究对象的空腹血糖(FBG)、总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白胆固醇(LDL-C)及高密度脂蛋白胆固醇(HDL-C),并采用酶联免疫吸附试验法检测其血清Omentin-1和MMP-9水平,将检测结果进行统计学分析。结果:(1)各组临床相关资料包括性别、年龄、BMI、FBG、TC、TG、LDL-C比较,差异无统计学意义(P0.05)。AMI组、SAP组及UAP组中HDL-C水平明显低于对照组,而收缩压、舒张压水平均高于对照组,差异均具有统计学意义(P0.05)。AMI组、SAP组及UAP中HDL-C.SBP、DBP水平相比较,无显著性差异(P0.05)。(2)AMI组、SAP组及UAP组中患者血清Omentin-1水平均显著低于对照组,MMP-9水平均显著高于对照组(P0.05),且AMI组患者血清Omentin-1水平明显低于SAP组及UAP组(P0.05),AMI组患者MMP-9水平明显高于SAP组及UAP组(P0.05)。(3)多支病变组血清Omentin-1水平低于双支病变组,双支病变组血清Omentin-1水平低于单支病变组,单支病变组血清Omentin-1水平低于对照组,差异均有统计学意义(P0.05);而多支病变组MMP-9水平高于双支病变组(P0.05),双支病变组MMP-9水平高于单支病变组(P0.05),单支病变组MMP-9水平高于对照组(P0.05);(4)血清Omentin-1水平与BMI、FPG、TG、TC、LDL-C呈负相关(相关系数r分别为-0.174、-0.254、-0.227、-0.141、-0.213),与HDL-C呈正相关(r=0.244,P=0.021),而与年龄、SBP、DBP均无明显的相关性(P0.05):(5)血清Omentin-1水平和MMP-9水平呈显著负相关(r=0.686,P0.05)。结论:血清Omentin-1水平与冠心病、冠心病的严重程度及斑块稳定性有关,有可能成为判断冠心病严重程度的指标之一。
[Abstract]:Objective: to investigate the relationship between serum omentin-1 and coronary heart disease, severity of coronary artery disease and plaque stability. Methods: from October 2013 to March 2014, patients with chest pain and palpitation were selected. 110 patients admitted to the Department of Cardiology of the first affiliated Hospital of South China University underwent coronary angiographic examination, including 90 patients with coronary heart disease and 20 patients in the control group. The sex, age and smoking history of 110 patients were recorded. 90 patients with CHD were divided into acute myocardial infarction (AMI) group (n = 28), stable angina pectoris group (n = 28) and unstable angina pectoris group (n = 34). According to the results of coronary angiography, the patients were divided into single vessel lesion group (n = 36), double vessel lesion group (n = 32) and multivessel lesion group (n = 22). Fasting blood glucose (FBG), total cholesterol (TC), triglyceride (TG), low density lipoprotein cholesterol (LDL-C) and high density lipoprotein cholesterol (HDL-C) were measured in 110 subjects. The serum levels of Omentin-1 and MMP-9 were measured by enzyme linked immunosorbent assay (Elisa). Results the clinical data of each group, including sex, age and age, showed that there was no significant difference in the level of HDL-C between SAP group and UAP group, but the systolic blood pressure (SBP) was significantly lower in SAP group and UAP group than in control group. The levels of DBP in SAP group and UAP group were significantly higher than those in control group. There was no significant difference in serum Omentin-1 levels between SAP group and UAP group. The serum Omentin-1 level in AMI group was significantly lower than that in control group (P 0.05), and the MMP-9 level in AMI group was significantly lower than that in SAP group and UAP group (P 0.05). The serum Omentin-1 level in SAP group and UAP group was lower than that in double vessel lesion group. The level of serum Omentin-1 in double vessel group was lower than that in single vessel group, and the level of serum Omentin-1 in single vessel lesion group was lower than that in control group. The level of MMP-9 in multivessel lesion group was higher than that in double vessel lesion group, the level of MMP-9 in double vessel lesion group was higher than that in single vessel lesion group, and the MMP-9 level in single vessel lesion group was higher than that in control group (P 0.05). The correlation numbers r were -0.174U -0.254U -0.227U -0.141U -0.213g, which were positively correlated with HDL-C 0.244P 0.021, but had no significant correlation with age SBPnP. There was no significant negative correlation between serum Omentin-1 level and MMP-9 level. Conclusion: serum Omentin-1 level is negatively correlated with coronary heart disease, and there is no significant correlation between serum Omentin-1 level and MMP-9 level. Conclusion: serum Omentin-1 level is negatively correlated with coronary heart disease (CHD), and there is a significant negative correlation between Omentin-1 level and MMP-9 level (P < 0.05), conclusion: serum Omentin-1 level has a negative correlation with coronary heart disease, and no significant correlation is found between Omentin-1 level and MMP-9 level. The severity of coronary heart disease is related to plaque stability, which may be one of the indexes to judge the severity of coronary heart disease.
