多排螺旋CTA成像在老年先天性椎动脉变异诊断中的价值

【摘要】：目的探讨多排螺旋CT血管造影(CTA)成像对老年先天性椎动脉变异的诊断价值。方法疑似先天性椎动脉变异老年患者150例均通过数字减影血管造影(DSA)诊断,以DSA诊断结果作为"金标准",研究先天性椎动脉变异病患实施多排螺旋CTA成像技术检测的情况,对比诊断符合率,并针对CTA影像特征进行观察。结果确诊131例椎动脉变异,其中63例单侧椎动脉先天性狭窄,47例单干椎动脉形成基底动脉,21例椎动脉行径变异。多排螺旋CTA诊断老年先天性椎动脉变异特异性89.47%(17/19),诊断敏感度为97.71%(128/131),准确性96.67%(145/150),阳性预测值达到98.46%(128/130),阴性预测值为85.00%(17/20),同椎动脉变异DSA诊断结果相比无统计学差异,利用Kappa一致性检验具备一致性。结论多排螺旋CTA技术诊断老年先天性椎动脉变异准确性良好,能够对椎动脉变异形态、特征做直观判断,减少漏诊或误诊风险。
[Abstract]:Objective to evaluate the diagnostic value of multi-slice spiral CT angiography (CTAA) in the diagnosis of congenital vertebral artery variation in the elderly. Methods 150 patients with suspected congenital vertebral artery variation were diagnosed by digital subtraction angiography (DSAs). The diagnostic results of DSA were used as the "golden standard" to study the detection of multiple spiral CTA imaging in patients with congenital vertebral artery variation, the diagnostic coincidence rate was compared, and the characteristics of CTA images were observed. Among 63 cases of congenital stenosis of unilateral vertebral artery, 47 cases were single stem vertebral artery and 21 cases were vertebral artery deviation. The specificity of multislice spiral CTA in diagnosis of congenital vertebral artery variation in the elderly was 89.47 / 19. The diagnostic sensitivity was 97.71 / 128 / 131, and the accuracy was 96.6775 / 150, Yang. The predictive value of sex reached 98.460.128 / 130%, and the negative predictive value was 85.00 / 20 / 20. There was no significant difference between the results of DSA diagnosis of vertebral artery variation. Conclusion Multi-row helical CTA technique has good accuracy in diagnosing congenital vertebral artery variation in the elderly, which can directly judge the shape and characteristics of vertebral artery variation and reduce the risk of missed diagnosis or misdiagnosis.
