实时三维超声成像技术对颈动脉粥样斑块体积测定的定量分析

【摘要】:目的:探讨实时三维超声成像(RT-3DU)技术用于定量分析颈动脉粥样硬化斑块体积的可行性及准确性。方法:对23例颈动脉粥样硬化患者行实时三维超声检查,对获取的35个斑块进行三维重建,所有斑块体积由2位超声医师进行随机双盲独立测定,1个月后,由其中1位医师再次进行测定。结果:RT-3DU定量测量的35个斑块的体积为(171±116)mm3。对于斑块体积的定量测量,两幅Bland-Altman图中仅见2.9%(1/35)的分散点在一致性界限外,两幅图差值均数的绝对值与差值均数为0的基线最大仅相差1.51mm3;对于管腔体积狭窄率的定量测量,两幅Bland-Altman图中仅见2.9%(1/35)的分散点在一致性界限外,两幅图差值均数的绝对值与差值均数为0的基线最大仅相差0.63%;2位观察者间及观察者内4组数据的组内相关系数均≥0.900,表示观察者间和观察者内部的一致性较高。结论:RT-3DU可以立体显示斑块的空间形态,斑块体积及管腔体积狭窄率的定量测量在观察者间和观察者内部均有较高的一致性,为动脉粥样硬化斑块的定性诊断和定量测量提供了新的检查手段。
[Abstract]:Objective: to investigate the feasibility and accuracy of real-time three-dimensional ultrasound (RT-3DU) technique for quantitative analysis of carotid atherosclerotic plaque volume.
