肺部感染致心律失常患者诊疗中心电图的应用研究

【摘要】:目的研究肺部感染致心律失常结果以及心电图检查在其诊疗中的价值,旨在及时控制感染,从而降低耐药菌产生、纠正心律失常,促进患者早日康复。方法选取2013年1月-2016年8月医院肺部感染心律失常患者138例,观察病原菌分布情况、心电图异常、心律失常类型以及感染控制前后心律失常变化情况,分析其间的关系。结果肺炎链球菌、肺炎克雷伯杆菌、铜绿假单胞菌、大肠埃希菌和金黄色葡萄球菌感染者,心电图ST-T改变率均高于异常P波、Q波和QS波(P0.05);肺炎链球菌、铜绿假单胞菌、大肠埃希菌感染者房性期前收缩率高于其他类型,肺炎克雷伯杆菌室性期前收缩率高于其他类型,金黄色葡萄球菌室性期前收缩、房性期前收缩率高于其他类型(P0.05);肺部感染控制后心律失常发生率为60.87%,低于控制前的100.00%(P0.05);窦性心动过速、室性期前收缩、房性期前收缩、束支阻滞和房室传导阻滞(Ⅰ型、Ⅱ型)发生率均低于控制前(P0.05)。结论肺部感染致心律失常以室性期前收缩与房性期前收缩最为多见,心电图以ST-T改变为主;加强心电图检查有助于掌握肺部感染者心律失常类型、评价病情,从而选取较为敏感抗菌药物及时控制感染、降低耐药菌产生。
[Abstract]:Objective to study the results of arrhythmia caused by pulmonary infection and the value of electrocardiogram examination in the diagnosis and treatment of pulmonary arrhythmia, in order to control the infection in time, so as to reduce the production of drug-resistant bacteria and correct arrhythmia. Methods 138 patients with pulmonary arrhythmia from January 2013 to August 2016 were selected to observe the distribution of pathogenic bacteria, abnormal electrocardiogram, type of arrhythmia and changes of arrhythmia before and after infection control. Results the ST-T change rate of Streptococcus pneumoniae, Klebsiella pneumoniae, Pseudomonas aeruginosa, Escherichia coli and Staphylococcus aureus was higher than that of abnormal P wave Q wave and QS wave P 0.05, Streptococcus pneumoniae. In Pseudomonas aeruginosa, Escherichia coli infection, the rate of atrial premature contraction was higher than that of other types, and the ventricular premature contraction rate of Klebsiella pneumoniae was higher than that of other types, and that of Staphylococcus aureus was higher than that of Staphylococcus aureus.
