坎地沙坦对高血压合并单纯高尿酸血症患者尿酸排泄的影响

【摘要】：目的:通过观察原发性高血压(EH)合并单纯高尿酸血症(HUA)患者,排除影响尿酸排泄的后天因素后,应用坎地沙坦前后,血尿酸、24小时尿尿酸、尿酸清除率的变化,判定坎地沙坦对尿酸排泄的影响,以帮助高血压合并高尿酸血症患者正确选择降压药物。方法:选择河北大学附属医院近16个月原发性高血压合并单纯高尿酸血症患者79例;所有受试者实验前后检测空腹血糖、血脂(胆固醇、甘油三酯)、血肌酐、尿PH值、24小时尿蛋白定量、尿常规。实验前检测肿瘤标记物、甲功五项、体质指数,筛选上述指标均在正常参考范围,体质指数在18.5-28之间的受试者。受试者实验期间均为低嘌呤饮食,饮水量2000ml/d,连续2周后,连续应用坎地沙坦3周,测定用药前后受试者血尿酸、24小时尿尿酸,计算尿酸清除率和校正尿酸清除率(尿量为2000ml标准的情况下计算得到的尿酸清除率)。结果:应用坎地沙坦3周后,患者血压水平下降,平均动脉压[治疗前(115.38±8.55)vs治疗后(98.04±6.55)mmHg,P0.01],血尿酸水平较治疗前无明显变化[治疗前(531.92±77.55)vs治疗后(529.31±83.05)umol/L,P0.05],校正尿酸清除率较治疗前无明显变化[治疗前(14.16±2.35)vs治疗后(14.31±2.50)ml/min,P0.05]。结论:坎地沙坦可降低平均动脉压,对尿酸排泄无明显影响。
[Abstract]:Objective: to observe the changes of uric acid and uric acid clearance rate in patients with essential hypertension (EH) complicated with hyperuricemia after removing the acquired factors affecting the excretion of uric acid before and after application of candesartan. To determine the effect of candesartan on uric acid excretion in order to help hypertensive patients with hyperuricemia to select antihypertensive drugs correctly. Methods: 79 patients with essential hypertension and simple hyperuricemia were selected from Hebei University affiliated Hospital in recent 16 months. Fasting blood glucose, blood lipids (cholesterol, triglyceride, creatinine, urine PH value, 24 hours urine protein quantification, urine routine) were measured before and after the experiment. Tumor markers, five items of thyroid function, body mass index were detected before and after the experiment. The subjects whose body mass index (BMI) was between 18.5-28 were all fed a low-purine diet with drinking water of 2000ml / d. After two weeks of continuous treatment, they were given candesartan for 3 weeks. Serum uric acid was measured before and after treatment, and the uric acid clearance rate and the corrected uric acid clearance rate were calculated. Results: after 3 weeks of application of candesartan, the uric acid clearance rate was calculated when the urine volume was the standard of 2000ml. The patient's blood pressure level drops, Mean arterial pressure [before and after treatment, 115.38 卤8.55)vs, 98.04 卤6.55mm HgG, P0.01], blood uric acid levels did not change significantly compared with those before treatment [529.31 卤83.05umolol / L] after treatment [531.92 卤77.55)vs], corrected uric acid clearance did not change significantly compared with pre-treatment [14.16 卤2.35)vs before treatment, 14.31 卤2.50 ml / min P 0.05].
