厄贝沙坦对老年H型高血压合并糖尿病患者靶器官的影响

【摘要】：目的探讨厄贝沙坦对老年H型高血压合并糖尿病患者靶器官的影响。方法选择2014年6月至2016年1月在郑州大学附属南阳市中心医院接受治疗的124例H型高血压合并糖尿病患者,按照随机数字表法将其分为观察组与对照组,各62例。对照组予以非洛地平缓释片治疗,观察组则予以厄贝沙坦治疗,比较两组患者治疗前与治疗1年后血压、血糖、靶器官损伤指标及靶器官损伤情况。结果经相应的治疗后,两组患者血压、血糖、血脂等指标均有不同程度的改善,且观察组收缩压变异性、舒张压变异性及Hcy水平明显低于对照组,高密度脂蛋白胆固醇(HDL-C)水平明显高于对照组,差异均有统计学意义(P0.05);观察组左心室质量指数(LVMI)、尿微量清蛋白/肌酐比值(ACR)较对照组明显降低,内生肌酐清除率(Ccr)较对照组明显升高,差异均有统计学意义(P0.05)。观察组左心室肥厚、肾损伤、脑损伤的发生率及存在靶器官损伤的患者百分比均低于对照组,差异均有统计学意义(P0.05)。结论厄贝沙坦对老年H型高血压合并糖尿病患者的靶器官改善作用较明显,且并发症较少。
[Abstract]:Objective to investigate the effect of irbesartan on target organs in elderly patients with H-type hypertension and diabetes mellitus. Methods 124 patients with type H hypertension complicated with diabetes were treated in Nanyang Central Hospital affiliated to Zhengzhou University from June 2014 to January 2016. The patients were divided into two groups according to the method of random number table: the observation group and the control group, 62 cases each. The control group was treated with felodipine sustained release tablet, the observation group was treated with irbesartan. The blood pressure, blood glucose, target organ damage index and target organ injury were compared between the two groups before and after treatment for one year. Results the blood pressure, blood glucose, blood lipid and other indexes were improved in the two groups, and the systolic pressure variability, diastolic blood pressure variability and Hcy level in the observation group were significantly lower than those in the control group. High density lipoprotein cholesterol (HDL-C) level was significantly higher than that in the control group (P 0.05), and the left ventricular mass index (LVMIN) and urinary albumin / creatinine ratio (ACRs) in the observation group were significantly lower than those in the control group. The creatinine clearance rate (CcrR) was significantly higher than that in the control group (P 0.05). The incidence of left ventricular hypertrophy, renal injury, brain injury and the percentage of patients with target organ injury in the observation group were lower than those in the control group (P 0.05). Conclusion Irbesartan can improve the target organ of elderly patients with H-type hypertension and diabetes mellitus with less complications.
