脉搏波传导速度和高同型半胱氨酸在老年高血压患者中的相关性研究

【摘要】：目的:探讨PWV和血Hcy在老年高血压患者中的关系,从而为老年高血压患者动脉硬化性疾病的早期识别提供有效的监测指标,为该病的早期防治提供新策略。方法:选择蚌埠医学院第一附属医院老年病科年龄在60岁以上的老年高血压患者86例。所有患者均符合《中国高血压防治指南》(2010修订版)的高血压诊断标准,并接受常规降压药物治疗,且治疗后经血压监测24小时平均血压小于140/90mmHg,排除严重的肝肾功能不全的患者。根据血Hcy水平将86例患者分为两组:正常组46例,血Hcy≤15μmol/L;升高组40例,血Hcy15μmol/L。分别测定两组患者的年龄、性别、体重指数(body mass index,BMI)、丙氨酸氨基转移酶(Alanine aminotransferase,ALT)、总胆固醇(total cholesterol,TC)、甘油三酯(triglyceride,TG)、肌酐(Creatinine,Cr)、空腹血糖(Fasting blood glucose,FBG)、糖化血红蛋白(HbA1c)、Hcy、收缩压(Systolic blood pressure,SBP)、舒张压(Diastolic blood pressure,DBP)、PWV等。结果:1.两组患者在性别构成、合并症等方面比较差异无统计学意义(p0.05)。2.高同型半胱氨酸组中的BMI、肌酐明显高于正常组(p0.05);两组患者的年龄、ALT、总胆固醇、甘油三脂、空腹血糖、糖化血红蛋白、收缩压、舒张压的比较差异无统计学意义(p0.05)。3.正常Hcy组和高Hcy组的Hcy水平分别为(11.73±2.28)μmol/L、(19.81±3.59)μmol/L,两组Hcy比较差异有统计学意义(p0.05);正常Hcy组和高Hcy组的bapwv水平分别为(1657.69±182.37)cm/s、(2013.43±248.91)cm/s,两组bapwv和Hcy比较差异有统计学意义(p0.05)。4.Pearson相关分析表明,bapwv与Hcy、年龄、BMI、ALT、总胆固醇、甘油三脂、肌酐、空腹血糖、糖化血红蛋白、SBP、DBP呈正相关(p0.05);多元线性回归分析表明bapwv的独立危险因素为年龄、胆固醇、甘油三酯、肌酐、Hcy、SBP等(p0.05)。结论:在老年高血压患者中bapwv的增快与高Hcy血症有正相关性,积极控制高同型半胱氨酸血症,对老年高血压患者的预后具有重要意义。
[Abstract]:Objective: to explore the relationship between PWV and blood Hcy in elderly patients with hypertension, so as to provide an effective monitoring index for the early identification of arteriosclerotic diseases in elderly patients with hypertension, and to provide a new strategy for early prevention and treatment of the disease.
