原发性高血压患者晨峰血压与血尿酸相关性研究

【摘要】:目的本研究通过对原发性高血压患者进行24小时动态血压监测及生化检验,探讨晨峰血压与血尿酸的关系,为原发性高血压患者的晨峰血压管理提供依据。方法入选2014年11月至2015年12月住院的原发性高血压患者(n=195),另入选同期健康体检者为正常对照组(n=88)。对高血压病患者行24h动态血压(24h ABPM)监测,根据监测结果将高血压病患者分为晨峰组(n=93)、非晨峰组(n=102);所有研究对象禁食12h后于动态血压监测当天早晨行尿酸、血糖、胆固醇、甘油三酯、高密度脂蛋白胆固醇、低密度脂蛋白胆固醇、尿素、肌酐检测,并记录一般临床资料。结果1.晨峰组、非晨峰组胆固醇水平均高于正常对照组,差异有统计学意义(P0.05)。2.24h ABPM显示:晨峰组全天、白天平均收缩压均保持较高水平,显著高于非晨峰组,差异有统计学意义(P0.05)。3.三组平均尿酸水平存在差异,晨峰组高于非晨峰组,两组均高于正常对照组,差异有统计学意义(P0.05)。4.相关分析提示晨峰血压与尿酸(r=0.384,P0.01)呈正相关;多元线性回归分析显示尿酸水平与血压晨峰呈独立正相关(B=0.05,P0.01)。结论1.血尿酸水平与高血压密切相关,是高血压的影响因素。2.高血压病患者中血尿酸与晨峰血压呈正相关,是晨峰血压的一个独立危险因素。
[Abstract]:Objective to investigate the relationship between morning peak blood pressure and serum uric acid in patients with essential hypertension by 24 hours ambulatory blood pressure monitoring and biochemical test, and to provide evidence for the management of morning peak blood pressure in patients with essential hypertension. Methods essential hypertension patients admitted from November 2014 to December 2015 were enrolled in the study. The patients were divided into morning peak group (n = 93) and non-morning peak group (n = 102). All the subjects were fasting for 12 hours and were given uric acid, blood glucose and cholesterol in the morning after fasting for 12 hours, according to the monitoring results, the patients with hypertension were divided into three groups: morning peak group (n = 93), non-morning peak group (n = 10) and control group (n = 12). Triglyceride, high density lipoprotein cholesterol, low density lipoprotein cholesterol, urea, creatinine were detected and general clinical data were recorded. Result 1. The cholesterol levels in morning peak group and non morning peak group were higher than those in normal control group, and the difference was statistically significant (P 0.05) .2.24 h ABPM showed that the mean systolic blood pressure of morning peak group remained high throughout the day, and significantly higher than that of non-morning peak group, the difference being statistically significant (P 0.05). The average uric acid level of the three groups was higher than that of the non-morning peak group, and the two groups were higher than the normal control group, the difference was statistically significant. Correlation analysis showed that there was a positive correlation between morning peak blood pressure and uric acid 0.384 (P 0.01), and multiple linear regression analysis showed that uric acid level was positively correlated with blood pressure (P 0.01). Conclusion 1. The level of serum uric acid is closely related to hypertension, which is the influencing factor of hypertension. Serum uric acid was positively correlated with morning peak blood pressure in patients with hypertension and was an independent risk factor of morning peak blood pressure.
