舍曲林联合心理干预对老年冠心病患者介入治疗后焦虑抑郁症状的影响及与血清IL-18和髓过氧化物酶水平的相关性

【摘要】：目的探讨舍曲林联合心理干预对老年冠心病患者经皮冠状动脉介入(PCI)后焦虑抑郁症状的改善情况及与血清白细胞介素(IL)-18、髓过氧化物酶(MPO)的相关性。方法 102例PCI手术伴有焦虑抑郁症状的冠心病患者102例随机分为对照组54例,采用常规治疗及给予安慰剂辅助治疗,观察组48例在对照组治疗的基础上给予盐酸舍曲林片,并实施心理干预。结果治疗后观察组汉密顿焦虑量表(HAMA)评分、汉密顿抑郁量表(HAMD)评分均显著低于对照组(均P0.05);观察组血清IL-18、MPO水平明显低于对照组(均P0.05);观察组血清IL-18、MPO与HAMA评分呈正相关(均P0.001),血清IL-18、MPO与HAMD评分呈正相关(均P0.001)。结论舍曲林联合心理干预治疗可明显改善介入治疗冠心病患者术后焦虑抑郁症状,术后IL-18、MPO检测可为患者的治疗和预后研究提供靶点。
[Abstract]:Objective to investigate the effect of sertraline combined with psychological intervention on anxiety and depression after percutaneous coronary intervention (PCI) in elderly patients with coronary artery disease (CHD) and its correlation with interleukin-18 (IL-18) and myeloperoxidase (MPO). Methods 102 patients with coronary heart disease associated with anxiety and depression in PCI operation were randomly divided into control group (54 cases) and placebo adjuvant therapy. 48 cases in observation group were treated with sertraline hydrochloride on the basis of control group. Psychological intervention was carried out. Results the Hamilton anxiety scale (Hama) score in the observation group after treatment was higher than that in the control group. The scores of Hamd were significantly lower in the observation group than those in the control group (all P 0.05), the levels of IL-18 MPO in the observation group were significantly lower than those in the control group (all P 0.05), and there was a positive correlation between the serum IL-18 MPO and the Hama score in the observation group (P 0.001), and a positive correlation between the serum IL-18 MPO and the Hamd score in the observation group (all P 0.001). Conclusion Sertraline combined with psychological intervention can significantly improve anxiety and depression symptoms in patients with coronary heart disease after interventional therapy. The detection of IL-18 MPO can provide a target for the treatment and prognosis of patients with coronary heart disease.
