中国汉族人群乙醛脱氢酶2基因多态性与主动脉夹层相关性研究

【摘要】:研究目的1、研究中国汉族人群乙醛脱氢酶2(aldehydedehydrogenase2, ALDH2)基因多态性与主动脉夹层的相关性。2、探讨影响主动脉夹层发病以及住院期间主动脉夹层患者死亡的危险因素。研究方法采用病例-对照研究方法。入选研究对象中病例组为2014年5月~2015年5月间山东大学齐鲁医院收治的主动脉夹层患者51例,根据年龄、性别构成比以1:2比例选取非主动脉夹层对照组102例,根据住院病历获得研究对象的临床基线资料,包括年龄、性别、吸烟史、饮酒史、高血压病史、糖尿病病史、D-Di(D-二聚体)水平、血脂水平、主动脉综合征家族史、临床结局等项目,血脂水平包括总胆固醇(total cholesterol, TC)、甘油三酯(triglyceride, TG)、低密度脂蛋白胆固醇(low-density lipoprotein cholesterol, LDL-C)、高密度脂蛋白胆固醇(high-density lipoprotein cholesterol, HDL-C)。抽取主动脉夹层患者及对照组清晨空腹外周肘静脉血2m1,提取DNA,以PCR方法扩增目的基因,所得产物纯化后以基因测序的方法确定ALDH2基因型。根据临床结局将病例组分为死亡组、及非死亡(好转出院或转院)组。SPSS18软件进行数据处理,对两组间临床基线资料、ALDH2基因多态性等进行比较分析,采用Logistic多元回归分析校正混杂因素,计算比值比(odds ratio, OR)及其95%置信区间(95%Confidence Interval,95%CI)。结果1.主动脉夹层组与对照组比较,性别、年龄、吸烟史无明显统计学差异,而ALDH2野生型基因、高血压病病史水平均高于对照组(p0.05),糖尿病病史、血脂异常水平低于对照组(p0.05)。2.Logistic多元回归校正年龄、性别、糖尿病、高血压、吸烟史、饮酒史、血脂异常等因素后,野生型ALDH2基因患者发生主动脉夹层的OR值为2.72(95%置信区间为1.16~6.41)。3.死亡亚组与非死亡亚组间性别糖尿病、吸烟史、饮酒史、心血管病家族史、血脂等反面差异均无统计学意义(p0.05),而D-Di、主动脉夹层类型存在显著差异(p0.05)。结论1、野生型ALDH2基因(ALDH2*1/*1)是中国汉族人群主动脉夹层发病的危险因素。2、D-Di、主动脉夹层类型与主动脉夹层临床结局相关。
[Abstract]:Objective 1 to study the association of aldehydes dehydrogenase 2 (ALDH2) gene polymorphism with aortic dissection in Chinese Han population, and to explore the risk factors influencing the incidence of aortic dissection and the death of patients with aortic dissection during hospitalization. Methods A case-control study was used. Fifty-one patients with aortic dissection were selected from Qilu Hospital of Shandong University from May 2014 to May 2015. 102 cases of non-aortic dissection control group were selected according to age and sex composition ratio of 1:2. The clinical baseline data including age, sex, smoking history, alcohol consumption history, hypertension history, diabetes history (D-dimer) level, blood lipid level, family history of aortic syndrome were obtained. For clinical outcomes, blood lipid levels included total cholesterol (TC), triglyceride (TG), low density lipoprotein cholesterol (LDL-C), high density lipoprotein cholesterol (HDL-C), high density lipoprotein cholesterol (HDL-C). Two m1s of fasting peripheral venous blood were extracted from patients with aortic dissection and control group in the morning. The target gene was amplified by PCR. The ALDH2 genotypes were determined by gene sequencing. According to the clinical outcome, the patients were divided into death group and non-dead group. SPSS18 software was used to process the data. The clinical baseline data of the two groups were compared and analyzed, such as the polymorphism of ALDH2 gene. Logistic multivariate regression analysis was used to correct the confounding factors and to calculate odds of odds (ORs) and 95% confidence interval (95% confidence interval). Result 1. There was no significant difference in sex, age, smoking history between aortic dissection group and control group, but ALDH2 wild type gene and hypertension history were higher than those in control group (P 0.05). The level of dyslipidemia was lower than that of control group (P < 0.05). 2. After adjusted age, sex, diabetes, hypertension, smoking history, drinking history and dyslipidemia, the OR value of aortic dissection in patients with wild-type ALDH2 gene was 2.7295% confidence interval was 1.16 / 6.41n.3. There was no significant difference in gender diabetes, smoking history, drinking history, family history of cardiovascular disease and blood lipid between the death subgroup and the non-dead subgroup, but there was significant difference in D-Diand aortic dissection type. Conclusion 1. Wild type ALDH2 gene ALDH2 1 / 1) is a risk factor for aortic dissection in Chinese Han population. The type of aortic dissection is related to the clinical outcome of aortic dissection.
