原发性高血压患者24小时尿钠钾排泄比与左心室质量指数的相关性研究

【摘要】:目的探讨原发性高血压患者24 h尿钠、尿钾和24 h尿钠钾排泄比与左心室质量指数(LVMI)的关系。方法选取2015—2016年新疆医科大学第一附属医院心血管内科住院的原发性高血压患者293例,留取其24 h尿标本,检测24 h尿钠、尿钾水平和24 h尿钠钾排泄比。依据24 h尿钠水平将患者分为:A组(尿钠≤100 mmol/24h),B组(100 mmol/24 h尿钠≤200 mmol/24 h)和C组(尿钠200 mmol/24 h)。进行心脏超声心动图检测,记录左心室舒张末期内径(LVEDD)、左心室收缩末期内径(LVESD)、室间隔厚度(IVST)、左心室后壁厚度(LVPWT)、左心室射血分数(LVEF)及LVMI。结果 3组患者IVST、LVPWT比较,差异均有统计学意义(P0.05);其中C组IVST大于A组和B组,C组LVPWT大于A组(P0.05)。Spearman相关分析结果显示,LVMI与24 h尿钠钾排泄比呈正相关(rs=0.119,P0.05),与24 h尿钠、尿钾无直线相关性(rs=0.139、0.087,P0.05)。多元线性回归分析结果显示,24 h尿钠钾排泄比是LVMI的影响因素[β(95%CI)=1.287(0.116,2.457),P0.05],24 h尿钠[β(95%CI)=-0.011(-0.050,0.027)]和24 h尿钾[β(95%CI)=0.071(-0.096,0.237)]与LVMI无明显相关性(P0.05)。结论高血压患者24 h尿钠钾排泄比与LVMI相关,独立于血压。
[Abstract]:Objective to investigate the relationship between 24 h urinary sodium, urinary potassium and 24 h urinary sodium potassium excretion ratio and left ventricular mass index (LVMI) in patients with essential hypertension.
