青海地区冠心病患者血清25-羟基维生素D水平与冠状动脉狭窄的相关性研究

【摘要】:目的维生素D在人体发挥着十分重要的作用,而且有多重生物学效应。近年来发现低水平的维生素D可增加心血管疾病、高血压及肥胖的发病风险,而冠心病是全世界最常见的疾病,故本研究的目的在于探讨25-(OH)D水平是否与冠心病患者冠状动脉狭窄相关。方法纳入2015年1月-2016年09月在青海省人民医院心内科住院的可疑冠心病患者共183例,其中男性103例,女性80例,平均年龄66.76±6.53岁,男女不限,该183例患者在行冠状动脉造影前均测定血清25-(OH)D水平。根据冠脉造影结果分为冠状动脉狭窄组和冠状动脉正常组,其中冠状动脉狭窄组共107例,男性58例,女性49例,平均年龄66.47±6.38岁,冠状动脉正常组共76例,男性45例,女性31例,平均年龄67.17±6.76岁,然后进行统计学分析。结果两组患者的性别、年龄、总胆固醇、甘油三酯、高密度脂蛋白、收缩压及舒张压等比较,差异没有统计学意义(P0.05),两组在LDL、25-(OH)D水平方面比较差异有统计学意义(P0.05);血清25-(OH)D水平与冠状动脉狭窄(Gensini评分)之间呈明显负相关,低密度脂蛋白水平与与冠状动脉狭窄(Gensini评分)之间无相关性。结论血清25-(OH)D水平与冠状动脉狭窄呈明显负相关,除目前已确定的冠心病的危险因素之外,血清25-(OH)D可作为冠心病的另一个重要危险因素。
[Abstract]:Objective Vitamin D plays an important role in human body and has many biological effects. Low levels of vitamin D have been found in recent years to increase the risk of cardiovascular disease, hypertension and obesity, and coronary heart disease is the most common disease in the world. The aim of this study was to investigate whether 25-(OH) D level is associated with coronary artery stenosis in patients with coronary heart disease. Methods from January 2015 to September 2016, 183 patients with suspected coronary heart disease were enrolled in Department of Cardiology, Qinghai Provincial people's Hospital, including 103 males and 80 females, with an average age of 66.76 卤6.53 years. Serum 25-(OH) D levels were measured before coronary angiography in 183 patients. According to the results of coronary angiography, there were 107 patients in coronary artery stenosis group and normal coronary artery group. There were 58 males and 49 females with an average age of 66.47 卤6.38 years. There were 76 cases in normal coronary artery group, 45 males and 31 females. The average age was 67.17 卤6.76 years, and then statistical analysis was made. Results Sex, age, total cholesterol, triglyceride, high density lipoprotein, systolic blood pressure and diastolic blood pressure were compared between the two groups. There was no significant difference between the two groups (P0.05), there was significant difference in LDL25- (OH) D level between the two groups (P0.05), there was a significant negative correlation between serum 25- (OH) D level and coronary artery stenosis (Gensini score). There was no correlation between LDL level and coronary artery stenosis (Gensini score). Conclusion Serum 25- (OH) D level is negatively correlated with coronary artery stenosis. In addition to the established risk factors of coronary heart disease, serum 25- (OH) D may be another important risk factor for coronary heart disease.
