非瓣膜病心房颤动患者生活质量评价及左心耳封堵术对其影响

【摘要】：目的:了解非瓣膜病心房颤动患者生活质量情况及左心耳封堵术对其生活质量的变化有无影响。方法:选取东南大学附属中大医院2014年9月至2016年6月心内科住院期间的非瓣膜病房颤患者100例为房颤组(其中11例行经皮穿刺左心耳封堵术),同期东南大学附属中大医院门诊体检的非心房颤动人群100例为对照组,使用汉密尔顿焦虑/抑郁量表(HAMA/HAMD)和美国简明健康测试量表(SF-36)对入选对象进行评分,11例左心耳封堵术患者在术后3个月再次进行生活质量评分。数据采用SPSS 13.0软件分析。结果:(1)房颤组患者八个维度生活质量相关评分比对照人群低(63.04土8.51vs.69.72±7.69、58.25± 18.14vs.69.70± 15.12、66.10± 12.52vs.71.30土 12.18、57.95土 13.91 vs.66.25士 12.78、55.65±11.93vs.62.40±12.28、68.69±14.85vs.75.80±11.09、67.69土21.94vs.72.36±15.74、70.86±15.90vs.77.96±9.56),其中七个维度差异有统计学意义(p0.05)。(2)房颤组中存在焦虑或抑郁情绪的患者占42%,对照组中存在焦虑或抑郁情绪的患者占7%,两组比较有明显差异(p0.05)。(3)合并焦虑/抑郁的心房颤动患者较单纯心房颤动患者八个维度生活质量评分更低(65.40±8.48vs.59.79土7.50、62.93±17.04vs.51.79±16.92、69.05±12.90vs.62.02±10.88、62.14土13.97vs.54.91±13.12、58.79±12.82vs.51.43±8.92、72.05±14.15vs.64.57士14.79、72.44±19.87vs.62.72±22.34、74.28±16.04vs.66.14±14.63),差异有统计学意义(p0.05)。(4)行左心耳封堵的非瓣膜病心房颤动患者术后三个月八个维度生活质量评分较术前升高(59.09±6.89vs.61.82±7.26、54.55±15.08vs.75.00±15.81、62.27 ±6.47vs.68.64±6.74、63.18±10.31vs.68.64±8.69、51.36±8.97vs.67.27±8.47、73.76±12.46vs.86.89±4.49、75.78±15.55vs.78.81±16.80、76.36±6.31vs85.46±8.63),其中五个维度有显著升高(p0.05)。结论:非瓣膜病心房颤动患者较非心房颤动人群焦虑、抑郁障碍发生率高,总体生活质量低,合并焦虑/抑郁障碍的心房颤动患者生活质量更低。左心耳封堵术可改善非瓣膜病心房颤动患者生活质量。
[Abstract]:Objective: to investigate the quality of life (QOL) in patients with non-valvular atrial fibrillation (NAF) and the effect of left atrial appendage closure (LASA) on the quality of life (QOL). Methods: from September 2014 to June 2016, 100 patients with non-valvular fibrillation were selected as atrial fibrillation group (11 of them underwent percutaneous transcatheter closure of left atrial appendage). 100 cases of non-atrial fibrillation patients who were examined in outpatient clinic of the middle and large hospitals were the control group.
