血浆半乳糖凝集素-3水平与慢性心力衰竭的相关性研究

【摘要】:目的:通过监测慢性心力衰竭(CHF)患者血浆半乳糖凝集素-3(Gal-3)及N末端B型利钠肽原(NT-proBNP)的水平,探讨血浆Gal-3水平在CHF中的临床价值,进而指导早期诊断CHF、评估病情严重程度及判断预后。方法:选取CHF(纽约心脏协会心功能分级II-IV级)患者120例及对照组40例,抽血检测所有入选研究对象的肝肾功能(AST、ALT、CR、BUN)、甘油三酯(TG)、总胆固醇(TC)、低密度脂蛋白(LDL-C)等常规生化指标。酶联免疫吸附法(ELISA)测定血浆Gal-3和NT-proBNP水平,超声心动图检查测定左室舒张末期内径(LVEDD)及左室射血分数(LVEF),收集并详细记录所有研究对象的一般资料,包括年龄、性别、BMI、吸烟及饮酒史等,通过统计学分析及处理比较心衰组与非心衰组间差异。受试者工作特征曲线(ROC曲线)分析血浆Gal-3与NT-proBNP水平在CHF的诊断价值。结果:1.心衰组Gal-3水平显著高于对照组(P0.05),且随心功能分级的增加而升高;2.血浆Gal-3和NT-proBNP水平与LVEDD均呈正相关(r=0.766,P0.01;r=0.496,P0.05),与LVEF呈负相关(r=-0.634,P0.01);3.Gal-3对心力衰竭同样有较高的诊断价值;4.血浆Gal-3水平在CHF不同病因组间无统计学差异。结论:血浆Gal-3水平与CHF具有一定相关性,血浆Gal-3水平与左室功能和结构的变化相关,表明血浆Gal-3参与了CHF左室重塑的过程,Gal-3对慢性心力衰竭有较高的诊断价值。因此可以指导早期诊断CHF、评估病情严重程度及判断预后。
[Abstract]:Objective: to investigate the clinical value of plasma galactose agglutinin 3 (Gal-3) and N-terminal B-type natriuretic peptide (NT-proBNP) in patients with chronic heart failure (CHF) in CHF. Thus, early diagnosis and CHF, can be used to evaluate the severity of the disease and to judge the prognosis. Methods: a total of 120 patients with CHF (New York Heart Association Cardiac function Grade II-IV) and 40 controls were selected. The liver and kidney function (AST,ALT,CR,BUN), triglyceride (TG), total cholesterol (TC), low density lipoprotein (LDL-C) and other biochemical indexes were measured. The levels of plasma Gal-3 and NT-proBNP were measured by enzyme-linked immunosorbent assay (ELISA), left ventricular end-diastolic diameter (LVEDD) and left ventricular ejection fraction (LVEF),) were measured by echocardiography, and the general data, including age, were collected and recorded in detail. BMIs, smoking and drinking history were compared by statistical analysis and treatment between CHF group and non HF group. The diagnostic value of plasma Gal-3 and NT-proBNP in CHF was analyzed by ROC curve. The result is 1: 1. The level of Gal-3 in heart failure group was significantly higher than that in control group (P0.05), and increased with the increase of cardiac function grade. The plasma levels of Gal-3 and NT-proBNP were positively correlated with LVEDD (P 0.01) and negatively correlated with LVEF (r-0.634 卤P0.01) 3.Gal-3 also had high diagnostic value for heart failure (P < 0.05). There was no significant difference in plasma Gal-3 levels among different etiological groups of CHF. Conclusion: plasma Gal-3 level is correlated with CHF, and plasma Gal-3 level is correlated with the changes of left ventricular function and structure, indicating that plasma Gal-3 is involved in the process of CHF left ventricular remodeling. Gal-3 has a high diagnostic value in chronic heart failure. Therefore, early diagnosis of CHF, can be used to evaluate the severity and prognosis of the disease.
