冠心病合并糖耐量减低患者中Lp(a)及hsCRP与胰岛素抵抗的相关性研究

【摘要】:目的探讨冠心病合并糖耐量减低患者脂蛋白(a)[Lp(a)]、超敏C反应蛋白(hs CRP)和稳态模型的胰岛素抵抗指数(HOMA-IR)的变化及其相互间的关系和意义。方法测定40例冠心病合并糖耐量减低患者(CHD+IGT组)和40例冠心病糖耐量正常患者(CHD组)及30例体检健康者(正常对照组)的Lp(a)、hs CRP等指标,用酶联免疫吸附测定法(ELISA)测定血清中Lp(a)、hs CRP;同时检测3组的空腹血糖、空腹胰岛素并计算HOMA-IR,稳态模型评估胰岛素抵抗指数(HOMA-IR)胰岛素抵抗程度采用稳态模式HOMA-IR进行评估。分析各组病例中Lp(a)、hs CRP指标的变化及其与HOMA-IR的关系。应用SPSS17.0统计学软件包进行统计分析,正态分布参数以平均数±标准差,非正态分布参数经对数转换,组间比较采用t检验,多组间比较采用方差分析;Lp(a)及hs CRP与HOMA-IR有关的因素作Pearson相关性分析。结果CHD+IGT组与CHD组及正常对照组比较Lp(a)、hs CRP、HOMA-IR均显著升高,差异有统计学意义(P0.05),CHD组与正常对照组比较Lp(a)、hs CRP、HOMA-IR显著升高,差异有统计学意义(P0.05)。Lp(a)、hs CRP均与HOMA-IR呈正相关。结论冠心病合并糖耐量减低患者Lp(a)、hs CRP表达及HOMA-IR明显升高,Lp(a)、hs CRP与HOMA-IR成正相关,Lp(a)、hs CRP及胰岛素抵抗共同参与了CHD合并糖尿量减低,Lp(a)、hs CRP及HOMA-IR在CHD合并糖耐量减低患者病情评估与治疗监测等方面有重要的参考意义。
[Abstract]:Objective to investigate the changes of lipoprotein (a) [Lp (a)], hypersensitive C-reactive protein (hs CRP) and insulin resistance index (HOMA-IR) in patients with coronary heart disease complicated with impaired glucose tolerance. Fasting insulin and insulin resistance index (HOMA-IR) were evaluated by steady-state HOMA-IR. The changes of Lp (a), hs CRP and their relationship with HOMA-IR were analyzed. The statistical analysis was carried out by SPSS17.0 statistical software package. The parameters of normal distribution were mean 卤standard deviation, the parameters of non-normal distribution were transformed by logarithm, and the t test was used to compare the parameters between groups. The analysis of variance (; Lp (a) and the factors related to hs CRP and HOMA-IR were used to analyze the correlation of Pearson. Results Lp (a), hs CRP,HOMA-IR in CHD IGT group was significantly higher than that in CHD group and normal control group, and the difference was statistically significant (P0.05 between), CHD group and normal control group), and the difference was statistically significant (P0.05). Lp (a), hs CRP was positively correlated with HOMA-IR). Conclusion the expression of Lp (a), hs CRP and the increase of HOMA-IR in patients with coronary heart disease complicated with impaired glucose tolerance are positively correlated with, Lp (a), hs CRP and insulin resistance, which may be involved in, Lp (a), hs CRP and HOMA-IR of CHD with impaired glucose and urine volume in CHD patients with impaired glucose tolerance. Evaluation and treatment monitoring have important reference significance.
