接触压力导管在零射线下消融阵发性房室结折返性心动过速中的应用

【摘要】:目的:本研究通过比较在零射线下使用压力导管(Smart Touch,Biosense Webster)与常规X线指导下使用普通消融导管消融阵发性房室结折返性心动过速(atrioventricular nodal reentrant tachycardial,AVNRT)来评价其安全性、可行性及有效性。实验方法:分析2015年10月至2016年9月在南方医院因AVNRT行零射线射频消融治疗的患者28例,即运用Carto3系统重建右心房三维模型后使用压力导管在零射线下行导管消融(实验组)。对照组是既往在南方医院因AVNRT行常规X线指导下导管消融治疗的患者30例。比较两组的消融功率、放电次数、手术即时成功率、手术总时间、X线曝光时间、并发症等。结果:两组的手术成功率均为100%;实验组与对照在手术总时间(实验组86.7±11.9 min VS对照组93.2±11.3min,P=0.37)、并发症上差别无统计学意义(P=0.37)。与对照组相比,实验组的消融功率(实验组25.7±6.OW VS对照组35.7±10.0W,P0.01)明显较低,放电次数(实验组4.8±1.1次VS对照组7.9±3.2次,P0.01)也明显较少,实验组全程零射线。结论:压力导管零射线消融AVNRT是安全有效可行的,全程零射线能避免射线对医患双方带来的危害。
[Abstract]:Objective: to evaluate the safety of paroxysmal atrioventricular nodal reentrant tachycardia (atrioventricular nodal reentrant tachycardial,AVNRT) by comparing the ablation of paroxysmal atrioventricular nodal reentrant tachycardia (atrioventricular nodal reentrant tachycardial,AVNRT) with the use of pressure catheter (Smart Touch,Biosense Webster) under the guidance of conventional radiography. Feasibility and effectiveness. Methods: Twenty-eight patients underwent zero-ray radiofrequency ablation (AVNRT) in Southern Hospital from October 2015 to September 2016 were analyzed. The three dimensional model of right atrium was reconstructed by Carto3 system and then catheter ablation was performed with pressure catheter (experimental group). The control group consisted of 30 patients who had been treated with conventional X-ray ablation under the guidance of conventional X-ray therapy due to AVNRT in Southern Hospital. The ablation power, discharge frequency, immediate success rate, total operation time, X-ray exposure time, complications were compared between the two groups. Results: the successful rate of operation was 100% in both groups, and the total operation time was 86.7 卤11.9 min VS in the experimental group and 93.2 卤11.3min in the control group. There was no significant difference in complications between the two groups (P0. 37). Compared with the control group, the ablation power of the experimental group was significantly lower than that of the control group (25.7 卤6.OW VS control group, 35.7 卤10.0WN P0.01), and the number of discharges in the experimental group (4.8 卤1.1 VS control group, 7.9 卤3.2 times) was significantly lower than that in the control group.
