ST段抬高性心肌梗死患者血运重建前后MMP-7及TIMP-1血清水平研究

【摘要】:[目的]探讨ST段抬高性心肌梗死(ST Segment Elevation Myocardial Infarction,STEMI)患者血运重建前后基质金属蛋白酶-7(Matrix Metalloproteinase-7,MMP-7)及组织型金属蛋白酶抑制剂-1(Tissue Inhibitor of Metalloproteinase-1,TIMP-1)血清水平的动态变化;同时探讨各时间位点血清MMP-7及TIMP-1浓度与左室功能及左室重塑之间的关系,为STEMI的诊疗提供新的思路和依据。[方法]1.选取2015年3月至2017年3月于我院心内科行急诊PCI的急性ST段抬高性心肌梗死患者38例,于PCI术前,术后24、48、72小时采集静脉血各4ml,3000r/min离心15分钟后,取上清,分装于1.5毫升EP管中,置于-80℃保存,后期用液相芯片技术(liquid chip,xMAP)检测各时间位点血清MMP-7及TIMP-1的浓度及其动态变化。2.在出院前对纳入的38位STEMI患者行超声心动图(ultrasound cardiogram,UCG)检查,测量左室射血分数(LVEF),左室收缩末容积(LVESV)、左室舒张末容积(LVEDV)等指标,评估其与各时间位点血清MMP-7、TIMP-1浓度的关系。[结果]1.随STEMI进程,血清MMP-7浓度从血运重建前(9.64±6.09ng/ml)至术后24-48h呈下降趋势(8.81±3.35ng/ml),而在术后48-72h迅速上升至峰值(10.23±7.71ng/ml),但差异无统计显著性。2.随STEMI进程,血清TIMP-1浓度在血运重建前即处于波谷(116.95 ± 56.11ng/ml),而后 0-48 小时呈上升趋势(135.18±59.57ng/ml),48-72 小时呈下降趋势(128.29±41.93ng/ml),但差异无统计显著性。3.随STEMI进程,MMP-7/TIMP-1比值在PCI术前达波峰,在0-48h逐渐下降达波谷,在48-72h呈上升趋势,推测MMP-7/TIMP-1比例失调可能参与急性心肌梗死的形成及心梗后的心室重塑,但差异无统计显著性。4.STEMI患者血运重建前后MMP-7、TIMP-1血清水平与LVEF、LVEDV、LVESV差异均不具统计学意义,均未见明显相关性。5.STEMI患者血运重建前后MMP-7、TIMP-1血清水平与肌钙蛋白I之间差异不具统计学意义,均未见明显相关性。6.STEMI患者血运重建前后MMP-7、TIMP-1血清水平与GRACE评分及SYNTEX评分之间差异不具统计学意义,均未见明显相关性。7.STEMI患者血运重建前后MMP-7、TIMP-1血清水平与甘油三酯、总胆固醇、HDL及LDL之间差异均不具统计学意义,均未见明显相关性。8.STEMI患者血运重建前后MMP-7、TIMP-1血清水平与血小板计数之间无明显统计差异,未见明显相关性。[结论]1.STEMI血运重建前后短时间(0-72h)内,MMP-7血清水平在0-48h呈下降趋势,48-72h呈上升趋势;而TIMP-1血清水平在0-48h呈上升趋势,48-72h呈下降趋势;MMP-7/TIMP-1比值在0-48h呈下降趋势,48-72h呈上升趋势;但MMP-7、TIMP-1血清水平以及MMP-7/TIMP-1比值在STEMI血运重建前后0-72h内无明显统计学差异。2.STEMI患者血运重建前后短时间(0-72h)内,MMP-7、TIMP-1血清水平与LVEF、LVEDV、LVESV未见明显相关性,推测在STEMI短时间(0-72h)内,MMP-7与TIMP-1比例失调暂不能引起左室结构和功能的变化,甚至出现心室重塑。
