冠心病合并糖尿病患者介入治疗后血清脂质过氧化物水平的变化

【摘要】:目的:观察冠状动脉性心脏病并患有糖尿病的患者,经桡动脉介入(PCI)术后脂质过氧化损伤指标的变化及冠脉病变进展情况。方法:选择38例冠心病患者(A组)及42例冠心病合并糖尿病患者(B组)进行PCI,分别在介入治疗前与治疗后即刻,30min,1h,24h及7d采血测定丙二醛(MDA)和超氧化物歧化酶(SOD)水平,并动态随访PCI术后冠状动脉病变的情况。结果:1.术后两组患者MDA与SOD水平均较术前有明显变化,脂质过氧化物指标明显变化,但这种变化在B组更加明显,至术后第7天,B组仍有明显变化,而A组已经接近术前水平。2.随访12个月发现,B组患者再狭窄,死亡,心血管事件发生显著高于A组(P0.05),卒中发生率无明显差异(P0.05),χ2值分别为4.20、4.21、5.17、0.30、22.33。结论:冠心病合并糖尿病患者PCI术后出现脂质过氧化损伤指标的变化持续且时间长,糖尿病可能促进了这一损伤过程。
[Abstract]:Aim: to observe the changes of lipid peroxidation injury and the progression of coronary artery disease in patients with coronary heart disease (CHD) and diabetes mellitus (DM) after transradial intervention (PCI). Methods: PCI, was performed in 38 patients with coronary heart disease (group A) and 42 patients with coronary heart disease complicated with diabetes (group B) before and immediately after interventional therapy. The levels of malondialdehyde (MDA) and superoxide dismutase (SOD) were measured at 24 h and 7 d, and the coronary artery lesions after PCI were followed up dynamically. Results: 1. The levels of MDA and SOD in the two groups were significantly different from those before operation, but the changes of lipid peroxides were more obvious in group B than those before operation. At the 7th day after operation, the levels of MDA and SOD in group B were still significantly changed, but the level of lipid peroxide in group A was close to the preoperative level. 12 months follow-up showed that restenosis, death and cardiovascular events in group B were significantly higher than those in group A (P0.05), and there was no significant difference in the incidence of stroke (P0.05). Conclusion: the changes of lipid peroxidation in patients with coronary heart disease complicated with diabetes mellitus after PCI are continuous and long. Diabetes may promote the process of this injury.
