原发性高血压患者左心室肥厚与同型半胱氨酸、心律失常的相关性

【摘要】:目的:根据原发性高血压(EH)伴左心室肥厚(LVH)病患的血清同型半胱氨酸(Hcy)值及心律失常程度的结果,探究血清Hcy的浓度、心律失常与左心室肥厚是否有关联,为EH伴左心室肥厚患者控制病情进程及早期干预提供可靠的理论依据。方法:依据纳入、各项排除标准,选取年龄段在45岁-70岁之间的EH患者80例(EH组),其中包含LVH组(40例)和不伴LVH组(40例),此外采选40例身体状况良好的人员设为对照组。分别对120例实验对象做如下处理:(1)记录下3次血压(BP)平均值、吸烟史、性别、身高、体重、年龄,计算出BMI数值;(2)抽取少量空腹静脉血,检测Hcy、LDL、HDL、TG、TC水平;(3)使用彩色多普勒超声探测仪测量心脏结构,计算出左心室质量指数;(4)行24小时动态心电监护记录心电变化,根据Kleiger、Lown分级方法统计心律失常的结果。根据三组研究对象的心律异常情况、血清Hcy浓度的对比结果,分析探讨这两者与EH伴左室肥厚有无关联。应用SPSS18.0进行数据的统计分析。结果:三组Hcy浓度的比较:(1)LVH组不伴LVH组健康对照组,组间相比后的差别都具备统计学意义P0.05。三组心律失常情况的比较:(1)总房性心律失常的差别无统计学意义(P0.05);(2)总室性心律失常、Kleiger及Lown分级≥3级中,肥厚组分别与不伴肥厚组及健康组的差异,符合统计学意义P0.05,不伴LHV与健康组之间的差别,无统计学意义P0.05。结论:从实验数据的结果中可以分析出:血清Hcy浓度、心律失常程度与LVH具有关联性。除去其他已知的LVH风险因子,Hcy浓度升高成为新晋的一员。LVH又是触发心律异常风险产生的紧要缘由之一。
[Abstract]:Objective: based on the results of serum homocysteine (Hcy) and degree of arrhythmia in patients with essential hypertension (EH) and left ventricular hypertrophy (LVH), to explore the relationship between serum Hcy concentration, arrhythmia and left ventricular hypertrophy. To provide reliable theoretical basis for EH patients with left ventricular hypertrophy to control the disease progression and early intervention. Methods: according to the inclusion and exclusion criteria, 80 patients with EH between 45 and 70 years old (EH group) were selected, including LVH group (40 cases) and non-LVH group (40 cases). In addition, 40 healthy persons were selected as control group. 120 subjects were treated as follows: (1) three times of blood pressure (BP) mean, smoking history, sex, height, weight, age, calculated BMI value; (2) a small amount of fasting venous blood was taken to detect the level of Hcy,LDL,HDL,TG,TC, (3) the structure of the heart was measured by color Doppler ultrasound, and the left ventricular mass index was calculated. (4) ECG changes were recorded by 24 hours dynamic ECG monitoring, and the results of arrhythmia were calculated according to Kleiger,Lown classification method. According to the arrhythmia of the three groups and the comparison of serum Hcy concentration, the relationship between them and EH with left ventricular hypertrophy was analyzed.
